s QI-045 Rev. 3 1.0 PURPOSE This procedure provides instructions and accept / reject criteria for the reinspection of structural steel.
2.0 APPLICABILITY This procedure applies to the reinspection of safety significant attributes of safety related hardware within the Structural Steel population at Comanche Peak Units 1, 2 and areas common to both units.

4.0 GENERAL 4.1 Responsibilities i
4.1.1 QA/QC Inspectors
(
QA/QC Inspectors perform reinspections in accordance with established project procedures and instructions.
Reference 3.2 addresses the method used to perform reinspections and to document and process the results.
4.1.2 QA/QC Discipline Engineers The QA/QC Discipline Engineers prepare the Quality Instruction delineating reinspection requirements and attributes.
4.2 Policy Activities performed under this procedure shall conform to the policies contained in the latest Comanche Peak Response Team Program Plan and Issue-Specific Action Plan VII.c.

For reinspection, the structural steel item is divided into three subcategories; A) member attributes, B) connection (s) attributes and C) field installed associated detail attributes. Checklists are prepared for each of the subcategories.

A.
Member Attributes A1.
Location Verify that the following attributes, where applicable, are located within the tolerance listed in table 5A to the location specified on the design drawings:

Record the material identification, i.e. unique vendor number, heat number and/or QP number on Attachment 6.2.

Field Performed Alterations The removal of material, i.e. cutting, notching, or coping, af ter the initial vendor fabrication, shall be considered a field performed alteration. Additionally, any member site fabricated shall be inspected for the following attributes.
(Site fabrication shall be determined by the QA/QC Discipline Engineer and noted in the remarks section of the checklist under the member attribute heading.)
a.
Location Verify that the location of all field performed alterations are within i 1/8" of the location specified on the design drawing, relative to the member, b.
Size Verify that the dimension (s) of the field performed alteration is less than or equal to the dimensions specified on the design drawings.
c.
Limit of Alterations Verify that the actual alterations are limited to those specified on the design drawings. Attachments to structural steel by other disciplines, i.e..
electrical cable tray supports, HVAC supports etc., shall not be considered an alteration and are outside the scope of this reinspection. However, should a support attachment be encountered during the reinspection, Attachment 6.15 l
shall be completed.
d.
Re-entrant corners l
Verify visually, that all re-entrant corners of field performed alterations are notch-free.

Cuts & Holes Verify that all field performed cuts & holes (excluding bolt holes) are without torn or 1
ragged edges.
B.
Connections For each connection to other building elements and member splices, perform the following inspections:

Location Verify that the location of the connection component work. points, other than bolts / anchors and/or welds (see sections B4 through B7) relative to the member, is within 1/8" of the location specified on the design drawings.

Size Verify that the connection components, other than bolts / anchors and/or welds (see sections B4 through O
B7), are as specified on the design drawings. For structural shapes and plate use the nominal dimensions and the standard mill tolerances listed in References 3.3 and 3.4.
Components cut from stock shall have a tolerance of i 1/8" for length and width and + 1/8", -0 for thickness.

Orientation Verify visually, that the connection is orientated, j
relative to the member, as specified on the design drawing.

Field Welds l
Prior to the start of inspection assure that welds are clean of any foreign material which would impede an adequate visual inspection. Coatings need not be removed unless an inspector feels that due to excessive and/or uneven coating application he is unable to I
provide an adequate inspection of the weld. Then he shall contact the level III inspector and with his concurrence only then may the coating be removed.
i Where it is necessary to measure weld dimensions during the course of an inspection, such dimensions should be determined as accurately as is practically achievable using standard weld inspection tools.

For welds identified as field welds on the design drawings perform the following tasks.
a)
Location Verify, visually, that the weld location is as specified per the design drawing.
b)
Weld Size Verify that the weld size, meets the requirements of the applicable drawings except that:
Fillet welds may be undersized to a maximum of 1/16" provided that the undersized portion (s) do not exceed 1/4 of the weld length.

For bevel welds which are inaccessible from the back side, the L
weld size shall be verified by assuring that the joint is welded flush except as noted in Attachment 6.13.

Fusion Verify that if any incomplete fusion is visible it complies with the following:
For fillet welds 3/8" maximum incomplete fusion in any 4" segment and 1/4" maximum incomplete fusion in welds less than 4" long.
For groove welds no incomplete fusion is permitted.
e)
Craters Verify that the weld size is met on all welds with underfilled craters. Craters outside the specified weld length are irrelevant provided there are no cracks visible through the coated surface if the weld has been O'
coated or on the uncoated surface if the coating has not been applied.
f)
Wald Profiles l
Verify, visually, that the weld profiles are acceptable per Attachment 6.13.
g)
Undercut Verify that any undercut present does not exceed the following criteria:
For base material 3/8" and less the following requirements apply:

When the member is welded from both sides and the undercut is in the same plane, the following undercut is permittc:.

For all other welds the permitted undercut is:
i) 1/32" for the full length of the weld, or
For base material greater than 3/8" the following requirements apply:
A)
For members welded on both sides, where the undercut is in the same plane the permitted undercut is 1/32" for 3/4 of the length of the veld and 1/16" for the remaining 1/4 of the length of the weld on both sides.
B)
For members welded only on one side or where the undercut is not in the same plane the undercut may be 1/16" for the full length of the weld.
h)
Porosity Considering that only surface porosity whose major surface dimension exceeds 1/16" shall be considered relevant verify the following:
The sum of the diameters of porosity do not exceed 3/8" in any linear inch of weld and shall not exceed 3/4" in any 12" of weld, and If 4 or more pores are aligned the edge to edge separation of the pores shall exceed 1/16".

Overlap If overlap is present verify that the required weld size and the fusion acceptance criteria are met.
If fusion in the overlap length cannot be verified then verify that the overlap length, O
whose fusion cannot be verified, does not exceed 3/8" in any 4" segment or 1/4" in welds less than 4" long.

Surface Slag Verify that the major surface dimensions of the slag does not exceed 1/8" or 1/4" for any isolated surface slag.
(Slag is considered to be isolated when it does not occur more frequently than once in a 3" weld segment.)

Cracks Verify that there are no cracks visible through the coated surface if the weld has been coated or on the uncoated surface if the coating has not been applied.

Welder ID Symbol When the welders identification symbol (s) is present, record the symbol (s) on Attachment 6.3.
If no symbol is present note " symbol not present" on Attachment 6.3.

Jam Nuts Verify, visually, that jam nuts are present when specified on the design drawings and for

high strength bolts with a nominal diameter greater than 7/8" in bearing type connections. In lieu of jam nuts, threads may be upset.
h)
Thread Engagement Verify, visually, that the end of the bolt is flush with or projects beyond the face of the nut.
When standard nuts are used as jam nuts (outer nut) they shall have a minimum thread t
engagement of one half the nut depth.

Richmond Inserts a)
Thread Engagement Verify the thread engagement for Richmond inserts is at least 2 times the bolt diameter plus l'/8 inch. Thread engagement shall be calculated as shown below using bolt length specified on the design drawing. Where bolt / stud length is not specified on the design drawing, the length shall be determined by an ultrasonic examination performed by Brown and Root in accordance with Reference 3.5.
This information shall be transmitted to the inspector by Brown and Root in writing. The inspector shall include this report in the reinspection package and based on this information verify the thread engagement per the sketches shown below.

B7.1 Size and Number of Anchors a)
Verify, by measurement, that the nominal anchor bolt diameter is as specified on the design drawing.
b)
Verify type, length and number of anchors installed match those shown on the Design Drawings.
Determine anchor bolt length by the letter designation stamped on the top of the bolt (Attachment 6.6).
Super kwik bolts are identified by a star in addition to the letter designation.
- If length identification is not verifiable or visible, an ultrasonic examination shall be performed in accordance with Reference 3.5 (Brown and Root shall perform UT and provide a copy of the results to the O

B7.2 Embedment Length Verify that the Hilti anchor bolt embedment plus the nut thickness (from Attachment 6.6) for the anchor being measured is not less then the "Embedment Length" or " Minimum Embedment Length" identified on the drawings; or lacking such information the " Minimum Embedment" identified on.6.**
Determine the Hilti anchor bolt embedment by measuring the projected length of the anchor bolt from the structural concrete
- surface.
Subtract the anchor projected length from designated anchor length to arrive at the Hilti anchor bolt embedment. The designated anchor length is that O
shown on Attachment 6.6 unless the stamp is indistinguishable in which case the U.T. recorded length should be used.
- NOTE:
In floor and roof areas where topping has been placed, the thickness of the topping shall be taken into account in determining the projected length. In these areas the thickness of the topping shall be added to measured length taken from the concrete surface. The inspector shall ensure that measurement for topping is provided on the inspection checklist for all roof and floor slabs.
- NOTE: When the letter designation stamped on Hilti bolt is not identified by Attachment 6.6, the following Hilti bolt attributes regarding length of Hiltis can conservatively be inspected by using Attachment 6.16.

Verify spacing of each Hilti to existing Richmond inserts, concrete edges, abandoned holes, Hilti anchors or embedded anchor bolts that have been cut off meet the minimum spacing requirements specified in Attachment 6.8 unless shown on design drawing, c)
Verify that the minimum distance of each Hilti anchor to any adjacent embedded plate meets the following requirements of

Attachments located within 12 inches of a Hilti anchor bolt shall have spacing as shown on Attachment 6.9.
Os For embedment plates without any attachment within 12 inch clearance distance, the anchor may be as close as practical to the edge of the plate without damage to the plate.
B7.4 Angularity Verify each Hilti anchor installed has its longitudinal axis within 6' from the perpendicular of the surface it is set into. Attachment 6.10 gives inspection requirements.
B7.5 Concrete Damage Verify structural concrete surrounding the fixture is not broken or spalled as a result of installing the Hilti anchor. If spalling exists, verify the depth does not exceed the following:

B7.6 Nut Engagement and Bearing a)
Verify nut of each Hilti anchor has proper engagement such that the end of the anchor bolt is not lower than the top of nut, unless otherwise shown on the Design Drawing.
b)
Verify nut and washer of each Hilti anchor has proper bearing and that a minimum of one washer (stendard or plate) has been provided. The washer shall completely cover the hole in the base plate. A level gauge shall be used for determining if the gap is less than 1:20 (Attachment 6.11).
c)
Verify that the nut for each anchor has not bottomed out.
Use thread length in.6 (or Attachment 6.16 if information is not shown on Attachment 6.6) and measured projection to determine acceptance.
d)
Verify no visible weld has been made to anchor or nut.
B7.7 Configuration a)
Verify that the location of the Hilti(s) is within 1/8", relative to the member, to the location specified on the design drawing.
b)
Verify that the edge distance, measured from the center of the Hilti, is equal to or greater than the dimensions specified on the design drawings. Should a dimension not be l
specified, the minimum edge distance (s) specified in section 1.16.5 of Reference 3.4 shall be followed.
B8.
Baseplates Verify by measurement that baseplates fit flush against the mating surface. The fc110 wing is acceptance criteria where baseplates are not flush:
The gap between the plate and concrete for a s
i maximum of 20% of the plate perimeter may exceed 1/16 inch, but at no point shall the gap exceed 1/2 inch.

If the above acceptance criteria is not met, the inspector shall, using a gage, map gap depth beneath the baseplate and determine if the area exceeding 1/16" gap is limited to less than 20% of the baseplate area. If verified, the attribute is acceptable.

Grout 4
Verify, visually,.that grout is installed when specified on the design drawings.

Field Installed Associated Details Field installed associated details include cover plates, side plates stiffeners, or other material which were added after j
the initial vendor fabrication. Additionally, any member which was site fabricated and includes any associated details shall be inspected for the following attributes.
(Site Fabrication shall be determined by the QA/QC Discipline Engineer and noted in the remarks section of the checklist under the field installed associated details heading.) These
( Os items may have been given a unique piece mark list for fabrication, however they are to be considered part of the main member for reinspection.
C1.
Location Verify that the location of the field installed associated detail, relative to the member, is within 1/8" of the location specified on the design drawing.
C2.
Size Verify that the size of each component is as shown on the design drawing. Use the nominal dimensions and the standard mill tolerances listed in Reference 3.3 or 3.4 for member size comparison. For items cut from stock the tolerance for width and/or length shall be i 1/8" and + 1/8", -0" for thickness.
C3.
Orientation Verify, visually, that the field installed associated detail is oriented, relative to the member, as shown on the design drawing.

Weld Verify, visually, welds in accordance with Section B4.

NOTES:
1.
Design drawings shall include; the G&H drawings and associated open design changes. Vendor and/or B&R erection / shop drawings, when included in.the reinspection package, are for reference and for dimensions and details not shown on the design drawings.
Should a conflict exist between the design drawings and the vendor /B&R drawings, the design drawings shall govern.
2.
Tolerances, if specified on design drawings, shall be used if there j
is a conflict with this procedure.
3.
Inaccessible items shall be indicated in the remarks column of the Reinspection Checklist.
4.
In the course of inspection the inspector shall note any item not covered by reinspection which appears out of the ordinary as O
related to the construction of the inspected item or surrounding area. Note such in the remarks column of the reinspection checklist.
I 5.
Inspector shall record the Tool Code (Attachment 6.14), serial number, calibration due date as applicable on Attachment 6.14.

Dimensions shall be measured to the nearest 1/16", unless otherwise specified.

Angles shall be measured to the nearest degree.
8.
Material identification may have been painted on the member, and would no longer be verifiable if the member has been field painted.
9.
Material for structural bolts shall be ASTM A325 unless specified otherwise on the design drawing.
- 10. All bolted connections are bearing type unless noted otherwise on the design drawings.
