This procedure ensures that the appropriate QA/QC Feview Team ,

personnel are certified, or otherwise meet the requirements of the '

Comanche Peak Response Team (CPRT). Moreover, it establishes requirements relative to indoctrination, examination, and training at the Comanche Peak Steam Electric Station (CPSES).

2.0 APPLICABILITY This procedure is applicable to those individuals implementing QA/QC Issue-Specific Action Plans (ISAPs) and any other individuals requiring training as determined by the QA/QC Review Team Supe rvisors.

3.0 REFERENCES

3.1 USNRC Regulatory Guide 1.58-Rev. 1, 1980 which endorses ANSI N45.2.6-1978, "Oualifications of Inspection, Examination and Testing Personnel for Nuclear Power Plants."

3.2 CIP-004, " Project Working Files."

4.0 GENERAL O The reported education and experience of every QA/QC Review Team member implementing QA/QC ISAPs is verified and determined to be commensurate with his or her assigned responsibilities.

Moreover, appropriate individuals undergo training relative to the CPRT activities and specific to their functions. Personnel performing inspections, examinations, and/or tests are certified in accordance with the requirements of this procedure.

4.1 Responsibilities 4

4.1.1 OA/QC Review Team Leader The QA/QC Review Team Leader (RTL) reviews Objectivity Questionnaires provided by the CPRT Senior Review Team and advises the CPRT Program Director as required.

The RTL also ensures that verifications of education / experience are conducted expediously, that the appropriate personnel meet any specific

i. education / experience requirements and advises the CPRT Program Director should an individual fail to meet those requirements.

CPP-003 R2 vision: 3 (3 4.1.2 QA/QC Certification Administrator

The QA/QC Certification Administrator (CA) ensures that each QA/QC Review Team member completes a CPRT Objectivity Questionnaire and advises the RTL should an individual not fully meet the established criteria.

The CA ensures that the education / experience of the appropriate QA/QC Review Team members are verified in a timely manner, compiled, and meet any applicable requirements.

The CA initiates action to indoctrinate appropriate QA/QC personnel in the responsibilities and objectives of the CPRT.

The CA ensures that inspectors continually meet established certification requirements and that certifications are suspended / reinstated as required.

The CA prepares and distributes the Record of Assigned Readings and coordinates training as required.

The CA maintains the certification / Qualification file, makes distribution as required, and transfers records to the QA/QC Records Administrator on a timely basis.

\- 4.1.3 ERC Level III Certifying Agent or QA/QC Inspection Supervisor The ERC Level III Certifying Agent or QA/QC Inspection Supervisor certify Level II inspectors.

1) be certified in accordance with the applicable requirements specified in Reference 3.1, 2) complete site specific training and tests addressed in this procedure, and 3) provide evidence of demonstrated proficiency as required by this procedure.

Should an activity be designated the responsibility of the RTL, it may be delegated by the RTL to another individual.

5.0 PROCEDURE 5.1 Confirmation of Objectivity 5.1.1 Initiation The CA shall ensure that a CPRT Objectivity Questionnaire is completed by each member of the QA/QC Review Team implementing QA/QC ISAPs.

5.1.2 Review and Approval Should any individual indicate a positive response on the questionnaire which requires further clarification, he or she is interviewed by the RTL. The RTL documents the results of such interviews which provide justification relative to the appropriateness of the individual to objectively implement QA/QC ISAPs.

Should any positive responses be unreconcilable, the

(') RTL prepares a memorandum to inform the CPRT Program Director.

The RTL forwards the memorandum relative to objectivity reviews to the CA for distribution.

5.1.3 Distribution and Control The CA distributes immediately the RTL's memorandum concerning objectivity reviews to the CPRT Program Director.

i The CA maintains a copy of all Objectivity Ouestionnaires and any documentation pertaining to 4

objectivity reviews in the secure Certification / Qualification file.

The CA forwards the Certification / Qualification file in a time y manner to the QA/QC Records Administrator to 1

ensure their distribution to the custodian of the Proiect Central File in accordance with Reference 3.2.

5.2 Verification of Education and Experience i

The CA shall ensure that the education and any applicable

/~' experience of QA/QC Review Team personnel implementing QA/QC ISAPs is verified in a timely manner.

1 CPP-003 l Rivision: 3 7-- 5.2.1 Initiation The CA ensures that letters similar to Attachment 6.1 are prepared, or telephone interviews to initiate the necessary verification are conducted. Telephone interviews are documented on a memorandum sinilar to Attachment 6.2.

Should verification be incomplete after 30 days of an individual's assignment at CPSES, or upon a date approved by the RTL, the CA shall advise the RTL by memorandum. The memorandum shall indicate the individual (s) involved, the action taken by the CA in order to obtain a timely response, address the cause of the delay, and specify the date the verification is scheduled for completion.

Upon receipt of the necessary information, the CA completes an Education / Experience Verification Summary in accordance with Attachment 6.3. Summaries completed by the CA are forwarded for review and approval to the RTL.

5.2.2 Review and Approval 9

Upon receipt from the CA, the RTL reviews

The RTL also reviews memorandums which address untimely verifications of education and/or experience to determine whether the cause of the delay and the scheduled verification date is reasonable. Should it be evident that the CA has taken measures to obtain a timely response, the RTL endorses the memorandum and denotes any instructions to the CA should the scheduled date be unacceptable, etc.

Personnel without the necessary education and/or experience are unauthorized to implement QA/QC ISAPs.

The RTL documents the results of each education /

experience review in a memorandum, then forwards it to the CA for control and transmittal to the Project Central File in accordance with Paragraph 5.2.3.

5.2.3 Distribution and Control

5.3 Indoctrination The CA ensures that QA/QC Review Team personnel implementing QA/QC ISAPs receive timely indoctrination in the responsibilities and objectives of the CPRT.

5.3.1 Documentation The RTL verifies that the appropriate personnel received indoctrination on a memorandum similar to Attachment 6.5. ,

5.3.2 Distribution and Control The CA maintains all Indoctrination Attendance Sheets in the secure Certification / Qualification file for transmittal to the Project Central File in accordance with Paragraph 5.1.3.

5.4 Inspector Certification 1

Personnel performing and/or conducting inspections, examinations, and/or tests are certified by discipline /sub-disciplines as shown in Table 5.4-1.

5.4.1 Initiation The CA shall obtain documentation pertaining to experience, schedule training, assign reading, and ensure that examinations and tests are conducted as required. Table 5.4-2 identifies the documentation required prior to inspector certification.

/~'s 5.4.2 General and Specific Examinations U- All ERC and those contract inspectors that do not have

  • specific site experience shall satisfactorily complete the General and Specific Examinations. The General and Specific Examinations are written and administered without the use of reference books, notes, etc.

Level III Inspectors, qualified in the applicable discipline /sub-discipline, review and approve all questions to be used in the General and Specific Examinations.

The General Examination need only be taken once.

Candidates are required to pass a Specific Examination for each desired level and discipline /sub-discipline.

However, it is not a requisite to pass a Level II Specific Examination to take a Level III Specific

, Examination for the same discipline /sub-discipline.

At least 80 percent of the questions must be answered correctly to pass either the General or any Specific Examinations. Should an individual fail an examination, he or she shall undergo training as described in Section 5.5, prior to reexamination.

Only one General and one Specific reexamination to i

initially qualify an individual is permitted. The reexanination shall be a new examination prepared from a reserve of questions selected randomly.

Reexaminations are not permitted before 3 working day s of the date the individual failed an initial qualification test.

5.4.3 Visual Acuity Examinations Personnel performing inspections, examinations, and/or tests shall undergo a Visual Acuity Examination at least annually. The CA ensures that Visual Acuity Examinations are documented on a Eye Test Certificate similar to Attachment 6.6.

5.4.4 Certification The Level III Certifying Agent or QA/QC Inspection

Should an inspector candidate fail to satisfy any of the applicable requirements addressed in this procedure, the certifying individual may qualify the Certification. Such qualification shall be noted in the " LIMITATIONS OF CERTIFICATION" portion of the ]

Personnel Certification form.

5.4.5 Distribution and Control The CA maintains all Personnel Certification Forms and any pertinent records, e.g., General and Specific Examinations results, in the secure Certification / Qualification file for transmittal to the Project Central File in accordance with Paragraph 5.1.3.

5.4.6 Certification Suspension The certifying organization shall suspend an individual's certification should that individual fail

a Visual Acuity Examination, incur a physical impairment that would adversely affect performance, or at the discretion of the QA/QC Review Team Leader.

The reason (s) for suspension shall be documented in a memorandum addressed to the individual's certification file and signed by the appropriate certifying individual.

The certifying organization shall reinstate an individual's certification when the cause of the suspension has been satisfactorily corrected.

Accordingly, documentation of the reinstatement shall be by a memorandum addressed to the individual's certification file and signed by the appropriate certifying individual.

5.5 Training QA/QC Re' view Team Supervisors determine the type and scope of l

training required for the members of their organization l including additional training if required by Paragraph 5.4.2.

Training is comprised of assigned reading, classroom training sessions, and/or on-the-job training.

,- , Rsvision: 3 5.5.1 Assigned Reading h(m The QA/QC Review Team Supervisors determine whether or not the members of their organization implementing ISAPs should read any material pertinent to activities at the CPRT, e.g., plans, procedures, instructions, reports, etc., and documents that determination in a memorandum to the CA.

Accordingly, the CA prepares a Record of Assigned Reading (similar to Attachment 6.8) which serves to document that the responsible individuals have read and comprehend the listed material.

5.5.2 Training Session Supervisors may recommend to the CA training to ensure that responsible personnel are completely knowledgeable of the intent of the specific requirements, their interaction with other requirements, and the method to implement those requirements.

Accordingly, the CA schedules and coordinates the necessary training. The CA also ensures that any required training material, e.g., projectors, slides, handouts, etc., are provided; that an instructor knowledgeable in the subject conducts the classroom session; and a Training Attendance Sheet similar to Attachment 6.9 is completed.

5.5.3 On-The-Job Training Supervisors advise the CA should it be appropriate to conduct training to develop and/or exhibit the techniques necessary to perform any specialized functions.

Accordingly, the responsible QA/QC Supervisor coordinates the necessary training; ensures that any required equipment, tools, etc. are provided; that an individual proficient in the procedure provides the training; and an On-The-Job Training Sheet similar to Attachment 6.10 is completed.

5.5.4 Distribution and Control The CA maintains all training records in the secure Certification / Qualification files for transmittal to the Project Central File in accordance with Paragraph 5.1.3.

2. Completion of college level work leading to an Associate Degree in a related discipline and 1 year of related experience in equivalent inspection, examination, or testing activities; OR
3. Completion of a 4 year college course resulting in a degree and 6 months of related experience in equivalent inspection, examination, or testing activities.

1. High school graduation / general education development equivalent and 10 years of related experience in equivalent inspection, examination, or testing activities; or high school graduation / general education development equivalent and 8 years related experience in equivalent inspection, examination, or testing activities with at least 2 years as Level II and at least 2 years associated with nuclear facilities, or in lieu of nuclear experience, indoctrination in the quality assurance aspects of a nuclear facility; OR
2. Completion of college work leading to an Associate Degree and 7 years of related experience in equivalent inspection, examination, or testing activities including at least 2 years experience associated with nuclear facilities, or in lieu of nuclear experience, indoctrination in the quality assurance aspects of a nuclear facility; OR

,i EYE TEST CERTIFICATION INSTRUCTIONS Visual Examination Requirements Eye examinations shall be given to inspection, examination, test and documentation review, personnel prior to certification and annually from the last date of examination. Current eye examinations provided by approved contractors for their personnel shall be considered acceptable. Subsequent eye examinations for contracted personnel shall be performed in accordance with the requirements of this Attachment.

A. Examiners Vision examinations shall be given by the Level III Certifying Agent, the Certification Administrator, an ophthalmologist, or an optometrist.

B. Vision Requirements Near Distance Visual Acuity The examination shall be given to assure natural or corrected near distance acuity in at 1 cast one eye. The applicant shall be capable

,/'N of reading a minimum of Jaeger No. 1 on a standard Jaeger Test Chart

\m / or equivalent. The ability to perceive an Ortho-Rater minimum of 10 or equivalent shall be considered an acceptable equivalent.

Color Vision Normal color vision shall be determined using Ishihara's Test or equivalent.

Should a candidate fail the color test, he or she may be given a

- practical color examination for specific inspection activities.

Candidates who demonstrate color vision applicable to the

? assigned inspections to be performed shall be considered to have acceptable color vision.

D. Restricted Certification Should the candidate fail to meet the above criteria, determine if the applicant can be certified to perform his job function under all applicable codes. Should the evaluation result in a restriction to the applicant's certification, indicate the restriction in the " Limitations of Certification" section of the Personnel Certification Form, (CPP-003.3).
