PREPARATION AND APPROVAL OF Revision: 0 DISCIPLitE SPECIFIC RESULTS 1 REPORTS Ato DAP COLLECTIVE EVALUATION REPORT l.0 PURPOSE The purpose of this procedure is to define the general format and content of the DAP Discipline-Specific Results Reports and the DAP Collective Evaluation Report. This procedure also defines the review and approval process of these reports within the DAP.

2.0 SCOPE This procedure applies to the preparation and approval of Discipline Specific Results Reports and the DAP Collective Evaluation Report. This procedure does not oddress the preparation of supporting documentation (such an engineering evaluation that are covered by DAP-8).

3.0 DEFINITIONS AND RESPONSIBILITIES 3.1 Definitions l 3.1.1 Discipline Specific Results Report j A Discipline Specific Results Report is that report required by the CPRT Program Plan to ossess overall DAP conclusions regarding the adequocy of design
within the subject discipline.

3.1.2 DAP Collective Evoluotion Report The DAP Collective Evoluotion Report is that report required by the CPRT Program Plan to assess in a collective fashion the adequocy of the design of Comanche Peak.

, 3.2.1 Discipline Coordinators Discipline coordinators are responsible for preparation of Discipline-Specific Results Reports. They may delegate the task of preparing draft portions of the results reports to others, but they shoII retain overall responsibility and shall sign the report as the " originator."

3.2.2 Design Adequacy Program Monoger The Design Adequocy Program Monoger is responsible for review and recommen-dotion of opproval of the Discipline-Specific Results Reports and for the

( perporation of the Design Adequocy Collective Evoluotion. He may delegate the task of drafting of portions of the Design Adequacy Collective Evoluotion to others, but shall retain responsibility for its preparation.

3.2.3 Review Team Leader The Review Team Leoder shall opprove the Discipline-Specific Results Report and the Design Adequocy Collective Evoluotion Report.

4.0 INSTRUCTION 4.1 Discipline-Specific Results Report l

l The Discipline Coordinator shall prepare (or cause to be prepared) the Discipline-Specific Results Report for his assigned discipline. The format and content of the report shall be consistent with Attochment A to this procedure unless TN-85-6262/9 Page 2 of S

! sheet (Attochment B) and forward the report to the DAP Monoger.

The DAP Manager shall review the Discipline-Specific Results Reports forward-ed by the Discipline Coordinators. His review shall consider whether the results report meets the requirements of this procedure, the CPRT Program Plon, and the Design Adequacy Plan. He shall also determine whether the scope of the report is consistent with the scope defined for the discipline by the process described in DAP-3. Furthermore, he shall evaluate whether the report is

internally consistent and consistent with supporting documentation such as checklists and engineering evoluotions. The DAP Monoger may delegate specific

He shall determine the extent and method of any further reviews. The Review Team Leader, DAP Monoger, and the Discipline Coordinator shall resolve any comments on the report. The Review Team Leoder shall then sign the report l cover sheet to indicate his opproval. The Review Team Leader shall forward the

! report to the CPRT SRT and resolve any SRT comments. Specific actions token as a result of SRT comments shall be documented by the Review Team Leader.

/ REPORTS IJO DAP COLLECTIVE EVALUATION REPORT 4.2 DAP Collective Evoluotion The DAP Manager shall prepare (or cause to be prepored) o DAP Collective Evoluotion that shall be consistent with the format and content of Attachment C unless otherwise approved by the Review Team Leader. U>on completion of the report, the DAP Manager shall sign the cover sheet (Attachment D) and forward the report to the Review Team Leader.

The Review Team Leader shall review (or cause to be reviewed) the DAP Collective Evoluotion. This review shall determine whether the Collective l Evoluotion is consistent with the Discipline Specific Results Reports and other Information developed by the DAP. The Review Team Leoder and the DAP Manoger shall resolve any comments. When he is satisfied with the report, the l Review Team Leoder shall sign the cover sheet to indicate his approval. The l Review Team Leoder shall forward the report to the CPRT SRT and resolve any SRT comments. Specific octions taken as a result of SRT comments shall be documented by the Review Team Leader.

S.0 DOCUMENTATION 5.1 Identification After gproval has been obtained, each Discipline-Specific Results Report shall be assigned a control identification number by the DAP Monoger. The identification number shall be of the following format:

DAP-CE-PM-001 The DAP Monoger shall enter all assigned Discipline-Specific Results Reports on' the Results Report Log (Attochment F). The log shall include the identification number, title, and revision level of all reports.

5.2 Retention The original copy of all Discipline-Specific Results Reports and of the Collective Evaluation shall be filed in the DAP files in occordance with DAP-14. The Review Team Leoder shall determine the distribution list for copies of the report.

2.0 SCOPE '

This section describes the scope of the report and, therefore, the scope of the design to which the conclusions are applicable. The interfoces between the discipline covered by this report and other disciplines should be addressed.

3.0 EXTERNAL SOURCE ISSUES 3.1 Description i

This section should describe the external issues covered by this results report and relate the issues to the action plon(s) that resolve them.

3.2 Methodology and Results l

This section describes the methodology applied to the resolution of the external source issues. It should describe the activities undertaken to evoluote the issue, the classification of the issue (observation, deviation, or deficiency), and the method and extent of third party octivities. Where generic imp;; cations or trending required expansion of the scope or opprooch from that presented in the action plan, o detailed description should be provided. This section may be organized in a manner deemed appropriate by the discipline coordinator. For example each issue could be treated as o separate subsection within 3.2 and a discussion prepored for each. Alternatively the section may be arranged by source of the issue (Cygno, CASE, NRC, etc.) or by grouping related technical issues.

3.3 Conclusions This section presents the conclusions found by the third party relative to external source issues only.

4.0 SELF-INITIATED REVIEW 4.1 Scope This section discusses the scope of the self-initiated review and the basis for that scope. Specifically it should oddress the evolution of that scope from that presented in Rev. 2 of the CPRT program pion, through the changes resulting from Phase 3 of the scope development procou and from implementation of the oction plan, to the final (Phase 4) scope.

4.2 Methodology i This section describes the methodology applicoble to the self-initiated review. It should concentrate on how the general requirements of the program plon were opplied to the specific discipline covered by the report. It is not necessary to repeat information contained in the i oction plans, however, enough information must be provided to allow o i reviewer (e.g. NRC or public) to obtain on understanding of the work 4 that was done. Where appropriate, lists of documents reviewed, I

references used, etc. should be provided.

4.3.1 Deficiencies

List all deficiencies found together with a description of the
deficiency, the corrective oction, root cause (if known), generic
implications, and other relevant dato.
