read:
"The RTL documents the results of such interviews on a form similar to Attachment 6.11."

Revise the second paragraph in Section 5.1.2 to read:
"The RTL forwards the form relative to objectivity reviews to the CA for distribution."
3.
Revise the first paragraph in Section 5.1.3 to read:
"The CA distributes immedi.;.41y the complete CPRT Objectivity Evaluation form to the CPRT Program Director."
4.
Add "6.11 CPRT Objectivity Evaluation Sample" to Section 6.0.
5.
Detach and include the CPRT Objectivity Evaluation Sample provided as Attachment 6.11.
