Gentlemen:

This refers to the inspection conducted by Messrs W. F. Smith and T.. R. Staker

- of-this office duriug the . period March 1 through April 15, 1989,- of activities

authorized by NRC Operating License NPF-38 for Waterford Steam Electric

Station,, Unit' 3, and to the discussion of our findings with Mr. J. R. McGaha

and other members.of your staff at the conclusion of the inspection.

. Areas' examined during the inspection included plant status, monthly maintenance

. observation, onsite followup of events, engineered safety feature system.

. walkdown, operational safety verification, monthly surveillance observation,

.and licensee event report. followup. Within these areas, the inspection

consisted of. selective examination of procedures and representative records,

interviews with personnel, and observations by the NRC inspectors. The

inspection findings:are documented in the enclosed inspection report.

During' this~ inspection, it was found that certain of your activities were in

vio1'ation of NRC requirements. Consequently, you are required to respond to

this-violation, in writing, in accordance with the. provisions of Section 2.201

of the NRC's," Rules of Practice," Part 2, Title 10, Code of Federal

-Regulations. Your. response should be based on the specifics contained in the

Notice of Violation enclosed with this letter.

In paragraph 4, we have documented a number of cases where errors were made

during engineering evaluations but were not caught during the checking and

review process. While it is understood that errors will be made, we expect the

review process to eliminate them to a greater degree than your staff appears to

have been demonstrating in the recent past. We further expect that you will

take action to improve this area of concern.

One unresolved item is identified in paragraph 4.d pertaining to multiple

problems associated with an inappropriate drawing change and its possible

~ impact on the operability of the Component Cooling Water System.

We have also examined actions you have taken with regard to previously

identified inspection findings. The status of these items is identified in

paragraph 8 of tne enclosed report.
