                               Identified in Inspection Report 87-15
          SUBJECT:   NRC Concern Over the Amount of Time That Effluent and Process
                     Radiation Monitors Have Been Out of Service
                                                                                                              l
          Root Cause:
                                                                                                              1
          The radiation monitoring system at Waterford 3 is a digitally based area
          and process radiation monitoring system. It consists of General Atomic                               '
           (GA) and Nuclear Measurement Corporation (NMC) radiation monitors. The
          system includes two computers for control of the system and over 80 micro
          processor controlled radiation monitors. The design is very complex and
          many problems have plagued it since startup. One reason many of these
          problems have not been resolved has been a lack of dedicated engineering
          support for the system. There has been no full-time system engineer for                             ,
          the radiation monitoring system. Lack of an engineer dedicated to the                               '
          radiation monitoring system resulted in a lack of engineered solutions to                           .
       .  long term problems.                                                                                 !
         Another reason problems with the radiation monitoring system have not been
          solved 1s lack of tra$ning and experience with radiation monitoring systems
         for LP&L I&C technicians. The workload necessary to keep the system

l operational and the number of I&C technicians assigned to maintenance of

          the system have not allowed for extensive training on radiation monitoring
         systems.
         Short Term Correction Action:
         LP&L has contracted a team of radiation monitoring system experts. The
         contract engineers will work with the recently aquired LP&L system
         engineer to investigate problems with the radiation monitoring system and
         implement corrective actions to improve system reliability. This includes
         analysis of failure history and trends on radiation monitors, providing
         solutions for recurring problems, and development of a tracking system for
         the radiation monitor system database and a basis for the numbers contained
         in the database. Health Physics and I&C Maintenance procedures for the
         radiation monitoring system will be reviewed and improvements made if
         required.
         The contract technicians are working with LP&L I&C technicians and
         providing them with valuable radiation monitoring system experience. The

i ' additional contract technicians also allow LP&L I&C technicians to be sent

         to vendor supplied training on General Atomic radiation monitoring systems.
         Long Term Corrective Ae' tion:
         LP&L will maintain a full time system engineer for the radiation monitoring
         system until such time as the system no longer requires this level of
         support. This individual would then be available as needed when new

         problems occur. This will ensure that upcoming problems will receive the
         engineering support necessary. LP&L will also train a group of I&C

                    purchase of. additional radiation monitoring system equipment to be.used as-

                    The.five NMC radiation monitors which are the least reliable monitors will
                    be replaced with General Atomic radiation monitors as' availability of
                    replacement units and plant schedules permit. ' Efforts are underway to
                    install as many of these as feasible during the second refueling outage.
                    This will.make the system uniform and make maintenance easier.
