Gentlemen:

This refers to the special inspection conducted by Mr. Paul Hartmann of

this office on January 23 through February 13, 1987, of activities at the i

Monticello Plant, authorized by License No. DPR-22, and to the discussion

of our findings with Mr. L. Eliason at the conclusion of the inspection.

This special inspection was initiated to review the circumstances surrounding

the isolation of a hydraulic control unit without verifying that adequate

shutdown margin was available with Control Rod 22-07 incapable of being

scrammed.

During this inspection, certain of your activities appeared to be in

violation of NRC requirements, as specified in the enclosed Notice. A

i written response is required.

With regard to the inoperable control rod on January 22, 1987, we are

concerned that your work request system permitted a control rod to be placed

out-of-service without proper evaluation. Therefore, please include in your

'

response to the violation your plans to review and upgrade your controls for

rendering equipment inoperable.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of

this letter, the enclosures, and your response to this letter will be placed

in the NRC Public Document Room.

The responses directed by this letter and the accompanying Notice are not ,

3 subject to the clearance procedures of the Office of Management and Budget  ;

as required by the Paperwork Reduction Act of 1980, PL 96-511.
