1 La Crosse Boiling Water Reactor Provisional License No. DPR-45 Response to Notice of Violation Cited in Inspection Report 50-409/88003 (DRP) l l

This letter is being submitted in response to your letter of j April 5, 1988 regarding NRC Inspection Report No. 50-409/88003 (DRP).

Included is the required written response to the identified violation.

NRC NOTICE OF VIOLATION Technical Specification (TS) 6.2.2 states: "The unit organization shall be as shown on Figure 6.2.2-1 ands a. Each on-duty shift shall be composed of at least the minimum shift crew composition shown in Table 6.2.2-1, Minimum i Shift Crew Compositions. The minimum shift crew composition for Operating Condition 4, Cold Shutdo.in, requires one Shift Supervisor, one Reactor Operator, and one Auxiliary Operator.

Contrary to the above, on January 29, 1988 for about 20 minutes, there was no Shift Supervisor onsite since the relief on-duty Supervisor )

inadvertently lef t the site for lunch at 1205 and the supervisor he was relieving did not arrive onsite until 1225 (LER 88-02).

Greennen IAC-12550 Page 2 April 29, 1988 DPC RESPONSE CORRECTIVE ACTION TAKEN AND RESULTS ACHIEVED At 1225 on January 29, 1988, it was noted by DPC personnel that the duty Shift Supervisor was absent without having been relieved. The oncoming supervisor immediately assessed the situation, obtained the necessary plant status from the Operators, and assumed the duty. When the absent supervisot returned to the site at 1250, a proper shift turnover was conducted.

CORRECTIVE ACTION TAKEN TO AVOID FURTHER NON-COMPLIANCE An Operations Review Committee Meeting was immediately ccavened, with the NRC Resident Inspector in attendance, to discuss the implications of the event. Following this meeting, the individual involved was counseled by the Plant Superintendent regarding attention to duty. On February 1, a memo was sent to all Shift Supervisors and Operators reminding ther of their continuf.ng responsibilities despite the plant being in a permanent shutdown mode. This memo also advised all Shif t Supervisors to do plant related paperwork in the Shift Supervisor's Office to the greatest extent possible while on duty.

Licensee Event Report No. 88-02 was submitted on February 18 addressing this incident.

DATE WHEN FULL COMPLIANCE WILL BE ACHIEVED Full compliance was achieved upon completion of tba formal shift turnover. Ensuring continued full compliance was completed upon distribution of the above referenced memo to the Shift Supervisors and Operators.
