Gentlemen:

This refers to the inspection conducted by liessrs. W. F. Smith and T. R. Staker

of this office during the period February 1-28, 1989, of activities authorized

by NRC Operating License NPF-38 for Waterford Steam Electric Station, Unit 3,

and to the discussion of our findings with fir. R. P. Barkhurst and other

members of your staff at the conclusion of the inspection.

Areas examined during the inspection included plant status, monthly maintenance

observation, operational safety verification, onsite followup of events,

monthly surveillance observation, and engineered safety feature walkdown.

Within these areas, the inspection consisted of selective examination of

procedures and representative records, interviews with personnel, and

observations by the NRC inspectors. The inspection findings are documented in

the enclosed inspection report.

During this inspection, it was found that certain of your activities were in

violation of NRC requirements. Consequently, you are required to respond to

these violations, in writing, in accordance with the provisions of

Section 2.201 of the NRC's " Rules of Practice," Part 2, Title 10, Code of

l Federal Regulations. Your response should be based on the specifics contained

in the Notice of Violation enclosed with this letter.

We are particularly concerned about your failure to comply with the Technical

,

Specification action statements associated with the inoperability of an

emergency diesel generator (EDG) when the supporting component cooling water J

system for the EDG is taken out of service for maintenance. This apparent

failure to comply with your Technical Specifications will be the subject of a

meeting between you and NRC Region IV at the Region IV office. The schedule

for this meeting will be established in a separate letter,

Additionally, we are concerned about the incident on February 15, 1989, where j

i

two of your employees operated a danger tagged valve contrary to the governing I

procedure. This is the second instance in one month where this has occurred.

The first was identified during the maintenance team inspection conducted late

January and early February 1989. It does not appear that you have adecuately

trained all site personnel to comply with all the provisions of the equipment

identified inspection findings. The status of these items is identified in

paragraph 8 of the enclosed report.

The response directed by this letter and the accompanying Notice is not :,ubject

to the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reduction Act of 1980, PL 96-511.

Should you have any questions concerning this inspection, we will be pleased to ,
