Gentlemen:

This refers to the inspection conducted by Messrs. W. F. Smith and T. R. Staker

of this office during the period August 1-31, 1989, of activities authorized by

NRC Operating License NPF-38 for Waterford Steam Electric Station, Unit 3, and -'

to the discussion of our findings with Mr. J. R. McGaha and other members of

your staff at the conclusion of the inspection.

Areas examined during the inspection included plant status, onsite followup of

events, monthly maintenance observation, monthly surveillance observation,

operational safety verification, and licensee event report followup. Within,

these areas, the inspection consisted of selective examination of procedures

and representative records, inter views with personnel, and observations by the

NRC inspectors. The inspection findings are documented in the enclosed

inspection report.

During this inspection, it was found that certain of your activities were in

'

apparent violation of NRC requirements. This apparent violation is discussed in

paragraph 4 of the enclosed inspection report and it involves the operation of .

a danger-tagged valve in violation of procedure requirements. This is the I

third instance this year where danger-tagged valves have been operated in

violation of procedural requirements. The two previous instances, which

occurred on January 31 and February 15, 1989, were cited as violations in '

inspection reports. Because of the similarity of these violations and in

accordance with Section V.B of the NRC's enforcement' policy, an enforcement

conference has been scheduled for October 11, 1989. in the NRC Region IV

office. This date was discussed with Mr. R. P. Barkhurst of your staff. At

this enforcement conference, we will discuss your understanding of this-

apparent violation, the reason for occurrence and your corrective actions. j

We will also want to discuss your evaluation of why LP&L's corrective actions  !

for the previous similar violations were not effective in preventing recurrence.

Following the enforcement conference, we plan a management meeting to discuss-

general topics including communications between the NRC and your staff.

We have also examined actions you have taken with regard to previously l

identified inspectior, findings. The status of these items is identified.in

paragraph 7 of the enclosed repnrt.
