LP&L Response to NRC Region IV Letter dated December 8, 1986

NRC request for additional information on -

Subject - Violation No. 8616-05, Part II, Electrical Panel Anchor

Bolts with Less Than Full Thread Engagement.

Reference - LP&L letter W3P86-2800 dated November 13, 1986 -

response to Violation 8616-05, Part II.

REQUEST: Provide assurance that there are no other anchor bolts for which

the nuts were installed with less than full thread engagement.

If other anchor bolts were incorrectly installed then an

appropriate evaluation of the seismic qualification of each

affected piece of equipment would be expected.

RESPONSE: The discrepancy that was the subject of the violation concerned a

concrete anchor bolt supporting electrical equipment that was

re-worked by Ebasco Force Account (EBFA) as a result of a

Non-Conformance Report (NCR). The NCR reported that the cabinet

was improperly mounted. To resolve the NCR the anchor bolts were

loosened and grout was placed under the cabinet. The bolts were

re-tightened after the grout had cured. As a result of this

rework a bolt was left with less than full thread engagement.

To provide assurance that all anchor bolts installed or reworked

for electrical equipment by EBFA are adequate, the remaining 226

EBFA electrical equipment installation packages were reviewed.

The purpose of this review was to identify the equipment setting

and bolting activities to determine if there may have been any

anomalies associated with equipment installation. Nine of these

226 work packages were related to the setting or bolting of the

electrical panels or cabinets.

In addition to the review of the packages, Operations QA/QC

conducted walkdowns on the nine installations relating to bolting

or setting activities.

SUMHARY OF WALKDOWN: The nine installation packages involved a total of 11

cabinets with 56 anchor bolts. Of these, one bolt

was found to have one thread less than full thread

engagement. An engineering evaluation determined

this condition to be acceptable.

CONCLUSION: The results of this review and associated field walkdown gives

an acceptable degree of confidence that the thread engagement

of cabinet mounting anchor bolts at Waterford 3 is fully

adequate for design conditions.
