SPECIAL REPORT DECEMBER 8,1986 FIRE BARRIERS BREACFED FOR LONGER THAN SEVEN DAYS

Purpose:

Technical Specification 3.14.5.b requires the submission of a report to the NRC if a non-functional fire barrier penetration is not returned to functional status within 7 days. In accordance with the above requirement, this special report on the breached barriers is being generated in order to notify the NRC. This report will be updated on a 30 day basis until the Appendix R associated work is complete.

Event:

The plant has been and is continuing to implement numerous modfications in accordance with Appendix R requirements. The schedular exemptions, which outlined the alternative compensatory measures to be taken when a barrier is breached, expired on September 30, 1986, and on October I,1986. Upon the expiration of the exemptions, the compensatory measures and reporting requirements of the Technical Specifications became effective.

Cause and Corrective Actions:

The major cause of the breached barriers is the Appendix R work which still remains to be completed. Due to the large volume of work being performed, the plant is reporting the breaches in this one report, rather than on a case-by-case basis. The breaches are grouped in three categories (see attached lists of breached barriers):

1) those breaches which are now closed, but were open for longer than 7 days,
2) those breaches which were open at the end of November, and were open for longer than 7 days at that time, and
3) those breaches which were reported as open in last month's report but were closed without exceeding the 7 day Technical Specification limit.

The plant continues to implement the compensatory measures as required by the Technical Specification.
