SPECIAL REPORT FEBRUARY. 23, 1989 FIRE PROTECTION SYSTEM IMPAIRMENTS REQUIRING NRC NOTIFICATION I l
l PURPOSE:
Technical Specification (TS) 3.14.5.b requires the submission of a report to the NRC if a non-functional fire barrier penetration.is not returned to functional status within 7 days. TS 3.14 also states that.certain other i conditions require NRC notification. In accordance with the above requirements, this special report is being generated in order to notify the NRC. !
l EVENT:
Fire Protection Impairment (FPI) Number 2254 was issued, on January 18, 1989, for -
rust holes through fire door 67-2. FPI 2257 was issued, on January 21, 1989, to repair thermolag insulation. 'I CAUSE AND CORRECTIVE ACTIONS:
The above impairments were caused by deterioration taking place over a period of time. Detector operability was verified and fire patrols were established, in accordance with Technical Specifications.

r FPI 2195, which identifies an impairment to flammastic, and FPI 2227, which identifies an impairment to barrier 75F, were previously reported as open.
FPI 2195 was closed on January 14, 1989, and FPI 2227 was closed on January 16, l 1989.
