SPECIAL REPORT EVENT On February 25, 1987, the Electric Driven Fire Pump (EDFP) exceeded the 7 day Technical Specification (TS) limit for the permissible time the pump may remain out of service (00S) without NRC notification. The EDFP was taken 00S at 0550 on February 18, 1987 to replace the pump packing. After the completion of maintenance, during testing of the pump, the outboard bearing overheated. The pump was shutdown and disassembled for inspection and repair. Upon disassembly, it was determined that several parts had undergone excessive wear and required replacement. Included in the parts to be replaced were the pump shaf t, shaft sleeves, bearings, and impeller.

After completion of the EDFP maintenance, the EDFP was tested satisfactorily. On March 4, 1987, at 1627, the EDFP was placed back in service. The actual 00S time exceeded the TS limit of 7 days.

CAUSE OF EVENT Several contributing causes to the event were identified, none of which by themselves would have resulted in the EDFP being 00S in excess of 7 days. The initiating event was the leakage of bearing grease past the grease seal in October, 1986. The bearing housing was removed and inspected. An unusually large amount of grease was pumped into the bearing housing, resulting in the leakage past the grease seal. The cccumulation of grease occurred because the Preventive Maintenance schedule calls for grease to be applied at a rate in excess of that at which the grease is being consumed. The accumulated grease was removed from the bearing and bearing housing, and the housing was then reinstalled. Following the reinstallation of the bearing housing, c grease gun was used to pump the housing with grease. When the gun's pressure gauge indicated that sufficient grease had entered the housing, the application was stopped. The impeller was also replaced at this time.

Following completion of maintenance, the EDFP was placed back in service.

On February 18, 1987, the bearing began to overheat. The bearing housing was removed, and no grease was found. A root-cause analysis performed by taintenance personnel has determined that the cause for the absence of any grease in the housing was the gauge indication suggesting that grease was entering the housing. In fact, the grease had stopped at the grease fitting in the bearing housing. Consequent bearing frilure was due to poor bearing lubrication. One of the components requiring replacement in February was the impeller. Following the initial impeller replacement in October, steps t

! were taken towards the issue of a purchase order for the procurement of a l spare impeller. Due to the required reviews and changes in vendor part i numbers which had to be resolved, the purchase order was not issued until

February 20, 1987. The impeller was received on February 21, however it l

l had several indications of casting defects and therefore required that an i engineering evaluation be completed prior to impeller installation.

4 The vendor subsequently confirmed that the indications were superficial casting defects which have no detrimental effect. These defects are normally ground out by the vendor but due to the time constraints placed on the order, this was not done. The engineering evaluation was completed on February 26, 1987. This evaluation recommended that the most severe indication be ground to determine if the defects were in fact surface defects. The impeller was ground and found to be acceptable for use on March 2, 1987.

4 ANALYSIS OF EVENT ll Turkey Point has two fire pumps: the EDFP, which is rated at 2000 gps, and a '

! Diesel Driven Fire Pump, rated at 2500 gpm. The pumps are supplied from two -

j Raw Water Tanks, each holding a minimum of 300,000 gallons of water.

l Either pump can draw water from either tank by opening a manual valve in l 1 the cross tie between the tanks. The Diesel Driven Fire Pump remained ,

) operational during the time that the EDFP was 00S.

COR1gCTIVE ACTIONS J

1) The shaft, shaft sleeves, and bearings were replaced.

]

2) The new impeller was installed after having its most severe surface defect ground out.

3) The techniques used for the gressing of bearings will be evaluated to i assure that proper quantities of grease are being injected into bearing 1 housings.

l 4) The site methods employed in the resolution of vendor exceptions to

purchase orders will be reviewed to provide better availability of spare parts through timely resolution of vendor exceptions. .
