IUIO ONH I Unit 3 operated at essentially 100% power until February 16. At 0438 hours0.00507 days <br />0.122 hours <br />7.242063e-4 weeks <br />1.66659e-4 months <br />, the Unit 3 turbine reheat, intercept, and control valves closed due to a loss of turbine control oil pressure. The reactor was manually tripped subsequent to high steam flow alarms, automatic control rod insertion, and loss of electrical load. During the event, both pressurizer Power Operated Relief Valves (PORVs) momentarily cycled, as expected for a loss of secondary load. A misaligned auxiliary governor test lever was bumped by a technician, causing control oil pressure to be dumped, and allowing the turbine valves to close.

Following the reactor trip a steam leak from the secondary system was discovered, caused by the failure of a 2 inch pipe upstream of a steam trap, on the Unit 4 AFW Train 2 steam supply line.

To isolate the steam leak, AFW Train 2 was secured. Despite the steam leak, the AFW system provided adequate feedwater flow to the steam generators for the duration of the event.

The loss of oil pressure was due to a misaligned test lever which caused the device to be sensitive to very small movement or minor bumping. The AFW steam leak was due to outside diameter (OD) corrosion resulting from water trapped under insulation.

The test lever has been repaired. All of the AFW steam piping has been pressure tested, and all of the piping susceptible to OD corrosion has been inspected. About 50 feet of 2-inch AFW steam piping (branch lines) has been replaced.

Unit 3 was returned to power on February 19, and remained above 80% power for the remainder of the month.
