Gentlemen:

Subject: Inspection Report No. 50-271/86-25

This refers to the routine resident safety inspection conducted by Mr. W. J. Raymond

of this office on November 4 - December 31, 1986, at the Vermont Yankee Nuclear

Power Station, Vernon, Vermont. The inspection consisted of observation of activi-

ties, interviews with personnel and document reviews. The results of the inspec-

tion are described in the NRC Region I Inspection Report enclosed with this letter

and were discussed with Mr. J. P. Pelletier of your staff at the conclusion of the

inspection.

Based on the results of this inspection, it appears that one of your activities

was not conducted in full compliance with NRC requirements, as set forth in the

Notice of Violation enclosed herewith as Appendix A. The violation concerns the

failure follow the requirements of the Section XI of the 1980 Edition of ASME Code,

in that the B core spray pump was not declared inoperable on two occasions per IWP-

3230 when test results fell within the " required action" range. This violation

has been categorized by severity level in accordance with the " General Statement

of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C

(Enforcement Policy 1986).

We understand that delays in procurement of the necessary replacement parts have

caused you to defer the inspection of the four residual heat removal (RHR) puups

until March, 1987. Equipment delivery problems aside, we also understand that you

were evaluating the need to defer the completion of inspections on 2 of the 4 pumps

due to budgetary reasons. Based on our review of this matter, we see no technical

justification for deferral of the planned inspections on all four pumps. We trust

you will proceed with the planned inspection in accordance with the commitments

made in your July 11, 1986 letter to NRC:NRR as expeditiously as possible, consis-

tent with material delivery schedules. If you are planning delays in inspections

of these pumps beyond March 1987, please let me know, in writing, including the

technical basis for such delay.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice of Violation when preparing your response. In

your response, you should document the specific actions taken and any additional

actions you plan to prevent recurrence.

The responses directed by this letter and the enclosed Notice are not subject to

the clearance procedures of the Office of Management and Budget as required by the

Paperwork Reduction Act of 1980, PL 96-511.
