Inspection Report No. 50-271/89-08
This refers to the routine safeguards inspection conducted by Messrs.
W. K. Lancaster, E. D. Sylvester and T. W. Dexter of Ois office on
May 15-19, 1989, of activities at the Vermont Yankee Nuclear Power Station,
Vernon, Vermont, authorized by NRC License No. DPR-28, and to the discussions
of our findings held by Mr. Lancaster with Mr. J. P. Pelletier and other
members of your staff at the conclusion of the inspection.
Areas examined during this inspection are described in the NRC Region I
Inspection Report which is enclosed with this letter.
Within these areas, the
inspection consisted of selective examinations of procedures and representative
records, interviews with personnel, and observations by the inspectors.
Based on the results of this inspection, it appears that several of your
activities were not conducted in full compliance with NRC requirements, as set
forth in the Notice of Violation, enclosed herewith as Appendix A.
These
violations have been categorized by severity level in accordance with the
" General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, - Appendix C (Enforcement Policy).
You are required to respond to this
letter and in preparing your response, you should follow the instructions in
Appendix A.
The response directed by this letter and the enclosed Notice is
not subject to the clearance procedures of the Office of M. nagement and Budget
as required by the Paperwork Reduction Act of 1980, PL 96-!.11.
The results of our inspection indicate a possible decrease in management
attention to the implementation of your security program, particularly with
respect to equipment and hardware. We are especially concurned that security
management was not aware of weaknesses in your perimeter i strusion detection
and assessment systems, which together could have permitted undetected access
into the plant protected area. These weaknesses represent a serious degrada-
tion of your progr9m effectiveness and should have been detected during routine
functional tests of the systems.
Therefore, your response should also address
why these weaknesses were not identified during routine functional tests of
both systems.

The results of m - inspection also indicate that a management meeting is
appropriate to discuss our findings further.
That meeting should be held
within 45 days of your receipt of this letter.
The additional time is being
provided to allow you to prepare a comprehensive plan to address the items
identified in this inspection report.
Please contact us after you have
reviewed this report to schedule that meeting.
Appendix A and certain paragraphs in the enclosed inspection report contain
details of your security program that have been determined to be exempt from
public disclosure in accordance with 10 CFR 73.21 (Safeguards Information).
Therefore, they will not be placed in the NRC Public Document Room and will
receive limited distribution. This letter and the remainder of the inspection
report will be placed in the Public Document Room, in accordance with 10 CFR 2.790(a).
