Subject: NRC Safety System Functional Inspection Team Report No. 50-271/90-80

This letter forwards the report of the safety system functional inspection

(SSFI) performed by an NRC team from August 6 to 17, 1990 at the Vermont

. Yankee Nuclear Station. The team's. findings and preliminary conclusions were

summarized for.you and members of your staff at the exit meeting held at the

. Vermont Yankee site on August 17, 1990.

In addition to the above, the team

performed reviews,and inspections,,from August 20 to 30. 1990 in the Region I

'

office, of the materials and information. that you provided to the team af ter

'

the onsiteLinspection.

Some members of the team'and the Division of. Reactor

'

Safety management also met with you at your request to receive; review, and

' discuss additional information that you had developed subsequent to the above

inspection regarding an unreviewed safety issue. -The team's finding and

. conclusions regarding this additional information have been summarized and

'

communicated to you by phone, and also by Mr. Chaudhary of my staff to

Mr. Pelletier of your staff at your Brattleboro, Vermont office on

The SSFI objective was-to assess the operational capability of the core spray-

-(CS) . system and the. service water portion of the residual heat removal: systems

,

to perform. their design basis safety functions. -- The team evaluated the adequacy

of operati,nal. procedures, test practices, and. maintenance policies as they

contributeLtothe-system'sreliability. The team also addressed the quality of

design control and other management programs applied to the systems. The team

-

concluded that the systems were capable of performing their design basis safety

,

functions.'

However, as discussed at the exit meeting, the inspection identified two

(

inappropriate applications of 10CFR 50.59, i.e., (1) core spray valve realignment

z

would better have been handled under a temporary exemption and, (2) Residual.

l

' Heat Removal Service Water. pump motor cooling-line redesign is subject to a

-

,

. single failure.

~In' addition, it appears that other activities were not conducted-

.}

in full' compliance with NRC requirements,;as set forth in the Notice of Violation

'

enclosed herewith as Appendix A.

The' violations have been categorized by severity

,

. level in accordance with-the " General Statement of Policy.and Procedure for NRC

Enforcement Actions," 10CFR Part 2, Appendix C (Enforcement Policy). You are

required to respond to this letter and in preparing your response, you should

follow the instructions in Appendix A.
