APPENDIX A NOTICE OF VIOLATION Vermont Yankee Nuclear Power Corporation Docket No. 50-271 Vermont Yankee Nuclear Power Station License No. DPR-28 Vernon, Vermont During the NRC inspection conducted on November 4 - December 31, 1986, a violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions", 10 CFR Part 2, Appendix C (En-forcement Policy 1986), the violation is listed below:

Technical Specification 4.6.E requires that inservice testing (IST) be con-ducted per Section XI of the ASME Code. As required by 10 CFR 50.55a(g)(4) and in accordance with Revision 8 of the Inservice Inspection Program (refer-ence: letter FVY 84-139, dated November 27, 1984), the licensee's IST program implements the 1980 edition of the ASME Code, Winter Addenda.Section XI, subsection IWP-3230 of this Code specifies that for deviations identified to fall within the Required Action Range of Table IWP-3100-2, the pump shall be declared inoperative and not returned to service until the cause of the de-viation has been determined and the condition corrected.

Contrary to the above, the results of IST testing of the B core spray pump completed on October 7 and November 25, 1986 and analyzed per IWP-3200 re-vealed pump vibration greater than 8 mils. This deviation fell within the Required Action Range of Table IWP-3100-2; however, no actions were taken to declare the pump inoperative or to enter the action statement of Technical Specification 3.5.A.2.

This is a Severity Level IV Violation (Supplement I.D.). f Vermont Yankee Nuclear Power Corporation is hereby requested to submit to this of-fice within twenty days of the date of the letter which transmitted this Notice, a written statement of explanation in reply, including (1) the corrective steps which have been taken and the results achieved; (2) corrective steps that will be taken to avoid further violations; and (3) the date when full conformance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
