Enclosed is NRC Report No.17, "Requal Tracking Report" from the Operator Licensing Tracking System. This report was used by the NRC to schedule requalification examinations for operators and record the requal pass dates. This report is similar to the " Active Operators Report" (Report No. 9) which is available in the PDR but the renewal dates were not listed chronologically. Report No.17 lists the effective expiration date according to month, date and year. We are using it to determine the number of renewal applications expected for a given year and we find this report may be of use to you. We also enclosed the facility license count summary from Report No. 9 for your information.
- Also, since we have discontinued sending the renewal notification letter, we are sending this report to each facility. The report will alert you at the beginning of the fiscal year when renewal applications, NRC-398 and Certification Of Medical Examination. NRC-396, are due. As you know, the Form NRC-396 and Form 398 should be sent to the NRC thirty days before the expiration date. Licenses terminated by your facility would have to be deleted from the report.
