Dear Mr. Maret:
On November 21,1998, the NRC cornpleted an inspection at your Vermont Yankee (VY)
reactor facility. In addition to the routine Resident inspector activities, announced inspections were conducted by NRC Region I specialists to assess aspects of the effluent controls program, corrective action for engineering open items and the motor operated valve test program. The enclosed report presents the results of that inspection.
During the six week period covered by this inspection period, routine activities were characterized by safe operation and generally conservative decision making. Also, the radioactive liquid and gaseous effluent control prograrns wore considered to be well-implemented. However, several personnel performance issues occurred during this period.
Although they were not significant in their impact, these issues are of concern. We understand you have initiatives onder way which are intended to improve human performance, in accordance with 10 CFR 2.790 of the NRC's " Rules of Practices," a copy of this letter and its enclosures will be placed in the NRC Public Document Room.
