Gentlemen:

Subject: NRC Region I. Inspection No. 50-271/89-02

This refers to the routine resident safety inspection conducted by Mr, G. Grant

of this office on February 14 - April 17, 1989, at the Vermont Yankee Nuclear

Power Station, Vernon, Vermont. The inspection consisted of observation of

activities, interviews with personnel and document reviews. The results of the

inspection are described in the NRC Region I Inspection Report enclosed with

this letter and'were discussed with Mr.' J. Pelletier of your staff at the

conclusion of the inspection.

Within the scope of this inspection, no violations were identified. However,

two areas-ef operations exhibited programmatic or performance weaknesses. As

detailed in this inspection report and others (IR 88-14, IR 88-19 and IR

88-20), your organization has experienced difficulty in effectively implemen-

ting corrective actions for a variety of deficiencies. Although subsequent

Lactions have apparently remedied the individual situations, initial inadequa-

cies in your corrective action program are of u me concern. Continuing manage-

ment attention and involvement in this area are needed. Another area discussed

in this report is control of plant activities. On balance, the VYNPC organi-

zation exhibited excellent overall coordination and control of outage activi-

ties. Effective communications and teamwork were evident whenever a compli-

cated or challenging evolution or work was in progress. However,. performance

, during a number of more routine activities indicai.ed occasional diminished

control. In most cases a' lack of attention to detail appears to have been at

the least a contributing factor. Although corrective actions for individual

problems appear to be adequate to prevent recurrence, continuing management

involvement is necessary to ensure. potential underlying common causes are

identified and corrected.

requesting a written assessment and any planned corrective actions pertaining.

to fire protection program deficiencies. Overall, your corrective actions

appear responsive to our concerns and adequately address the issue. Continuing

improvement in the fire protection program area is encouraged. The long-term

effectiveness of these actions will be evaluated during routine inspections.
