NRC INTEGRATED INSPECTION REPORT 50-271/98-14
Dear Mr. Maret:

On January 4,1999, the NRC completed an inspection at your Vermont Yankee (W) reactor
facility. The enclosed report presents the results of that inspection.
During the six week period covered by this inspection report, the conduct of activities at W was
generally characterized by safe operations and maintenance. However, the NRC's initial review
of the scram discharge volume drain valve failures indicates W's design change process was not
effective in this case. This issue, which may represent a violation of NRC requirements, will
remain open for a reasonable time to allow W to develop its corrective actions. In addition, the
failure to return a high pressure coolant injection system instrument to service, following
maintenance, is of concern because it reflects a continued problem with human performance.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practices," a copy of this letter and its
enclosure will be placed in the NRC Public Document Room.
