            Gentlemen:
            Thank you for your letter dated June 16, 1986, in response to our letter dated
            April 4, 1986. As a result of our review, we find we are in need of additional
            information with respect to Item A in the Notice of Violation (NOV).
            Specifically, your response failed to provide any actions taken with respect to
            the failure of the design review process to identify the technical inadequacy
            of Design Change Authorization 13,349. Additionally, we do not consider that
            reliance solely on personnel discussions provides an adequate basis for
            determining that the identified use of a grouted Richmond Insert was a single
            case. Accordingly, please describe those actions, planned or taken, which will
  i          substantiate the conclusions made from discussions with personnel.
            We have no further questions regarding Items B, C, and D in the NOV and Items A,
            B, and C in the Notice of Deviation. This determination is based, however, on
            all references in your letter to training sessions, meetings, and discussions
            being appropriately documented and available for audit. Please inform us if
            this is not fully the case.
