This refers to the special team inspection conducted by Messrs. A. T. Howell,-

D. L. Kelley, D. N. Graves, and R. B. Vickrey during the period July 31 through-

August 7, 1990, of activities authorized by NRC Facility Operating _

License NPF-87 and NRC Construction Permit CPPR-127 for the Comanche Peak Steam

Electric Station, Units 1 and 2, and to the discussion of our findings with you

and other members of your staff at the conclusion of the inspection.

This inspection was conducted to review the circumstances following a. safety

,

injection event at Comanche Peak Steam Electric Station (CPSES), Unit 1, on

'

July 30,1990.

The enclosed copy of our inspection report identifies areas examined during the

inspection. Within these areas, the inspection consisted of selective

examination of procedures and representative records, interviews with

personnel, and observations by the inspectors.

During this inspection, it was found-that certain of your activities were in

violation of NRC requirements. This violation would normally be considered as

noncited per 10 CFR 2, Appendix C, criteria; however, although your staff

identified the violation and promptly corrected it, this violation was

potentially more significant because the operability of all four atmospheric

relief valves could have been affected by the presence of water from isolated

main steam isolation valve (MSIV) upstream drippots.

Since action has already

been taken to correct the hot standby operating procedure inadequacies, no

'

response to this violation is required.

During this inspection, you committed in an Augus't 4, 1990, letter to NRC

Region IV (TXX-90283) to implement a design modification to remove the local

switches for the essential lights, as appropriate. We, thereby, request that

you provide, in writing, to NRC Region IV, a schedule for the implementation of

this design modification, as well as a description of any interim measures that

you deem appropriate to ensure that all essential lighting remains energized

until such time that the design modification can be implemented. We request

that you provide this information within 30 days of the date of this letter.
