Gentlemen:
This refers to the inspection conducted t.y Messrs. S. D. Bitter, R. M. Latta,
M. F. Runyan, A. T. Howell, D. N. Graves, and W. D. Johnson during the period
May 3 through June 5, 1990, of activities authorized by NRC Facility Operating
License NPF-87 and NRC Construction Permit CPPR-127 for the Comanche Peak Steam
Electric Station, Units 1 and 2, and to the discussion of our findings with you
and members of your staff at the conclusion of the inspection.
Areas examined during the inspection included plant status, operational safety
verification, onsite followup of events, monthly maintenance observation,
monthly surveillance observation, startup test witnessing, followup of licensee
event reports (LERs), followup on 10 CFR Part 50.55(e) deficiencies, licensee
plans for coping with strikes, and Unit 2 walkdowns. Within these areas, the
inspection consisted of selective examination of procedures and representative
records, interviews with personnel, and observations by the inspectors.
The
inspection findings are documented in the enclosed inspection report.
During this inspection, it was found that certain of your activities were in
violation of NRC requirements. These violations would normally be considered
as noncited per 10 CFR 2 Appendix C criteria; however, although 31ur staff
identified Violation A, the NRC identified a procedural weakness related to
this violation during the previous inspection period.
Had prompt corrective
action been taken at that time, Violation A may not have occurred.
Your staff
also identified Violation B; however, Violation B was only detected by chonce
and had Violation 8 not been detected before exceeding 50 percent rated thermal
power, operation of the atmospheric relief valves with reduced capacity may
have had additional safety significance.
Consequently, you are required to
respond to these violations, in writing, in accordance with the provisions of
Section 2.201 of the NRC's " Rules of Practice," Part 2, Title 10, Code of
Federal Regulations.
Your response should be based on the specifics contained
in the Notice of Violation enclosed with this letter.
During this inspection, we determined that there were no detailed, written
strike contingency plans or procedures which would delineate the required
operations department or support organization actions during a strike.
We,
thereby, request that you review this matter to assure that appropriate plans
are in place before the next strike negotiation deadline.
