Gentlemen:

This refers to the inspection conducted by Mr. W. D. Johnson and other members

of the Region IV staff during the period August 7 through September 18, 1990,

of activities authorized by NRC Facility Dperating License NPF-87 and NRC

Construction Permit CPPR-127 for the Comanche Peak Steam Electric Station,

Units 1 and 2, and to the discussion of our findings with you and members of

your staff at the conclusion of the inspection.

Areas examined during the inspection included plant status, operational safety

verification, onsite followup of events, maintenance observation, surveillance

'

observation, startup test results review, engineered safety feature system

walkdown, licensee event report followup, followup on previously identified

items, and Unit 2 activities. Within these areas, the inspection consisted of

,

selective examination of procedures and representative records, interviews with

personnel, and observation by the inspectors.

The inspection findings are

documented in the enclosed inspection report.

During this inspection, it was found that certain of your activities were in

violation of NRC requirements.

Consequently, you are_ required to respond to

these violations in writing, in accordance with the provisions of Section 2.201

of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal

Regulations. Your response should be based on the specifics contained in the

'

Notice of Violation enclosed with this letter.

In accordance with NRC enforcement policy, a Severity Level IV violation can be

considered for nonciting if appropriate criteria are met. Violation A was

cited in this report because failure to take the appropriate logs specified in

the alarm response procedure was NRC identified.

It is potentially significant

because the total time that an actual high flow condition should be allowed to

exist between eddy current inspections of the steam generator is 100 hours0.00116 days <br />0.0278 hours <br />1.653439e-4 weeks <br />3.805e-5 months <br />.-

Violation B was cited because it was identified by the inspector and it

represents two additional examples of failure to comply with safety-related

procedures.

In addition,-a violation of Technical Specification 3.4.8.2 was identified.

This involved a failure to properly evaluate pressurizer heatup transients.

However, in accordance with Section V.G 1 of the NRC's Enforcement Policy

(10 CFR Part 2, Appendix C), no citation is being issued and no written

response is required.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this

letter, the enclosures, and your response to this letter will be placed in the

NRC Public Document Room.

The response directed by this letter and the accompanying Notice is not subject

to the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reduction Act of 1980, PL 96-511.

discuss them with you.
