Dallas, Texas 75201 Gentlemen:- This refers to the inspection conducted t,;< Messrs. S. D. Bitter, R. M. 'Latta, , M. F. Runyan, A..T. Howell, D. N. Graves, h. F. Bundy, D. L. Kelley, and W. D. Johnson during the period June 6 through Aly 3, 1990, of activities authorized by NRC Facility Operating License NPF-Bi and NRC Construction Permit CPPR-127 for the Comanche Peak Steam Electric Station, Units 1 and 2, and to the discussion of our findings with Mr. H. D. Bruner'and other members of your staff at the conclusion of the inspection. - -Areas examined during the inspection included plant status, operational safety. verification, startup test results review, quality assurance for;the startup test pre ram, onsite followup of events, monthly maintenance observation, monthly surveillance observation, . startup test witnessing, followup of -li_censee event reports, followup of NRC Bulletins, Information Notices, Generic Letters, i and Unit 2 walkdowns. Within these areas, the inspection consisted of ' selective examination of procedures and representative records, interviews with personnel, and observations by the inspectors. The inspection findings are documented in the enclosed inspection report. During this inspection, it was found that certain of your activities were in , . violation of NRC requirements. Consequently, you are required to respond-to t L these violations in writing, in accordance with the provisions of Section 2.201 of the NRC's " Rules of Practice," Part 2, litle 10, Code of Federal , Regulations. Your response should be based on the specifics contained in the ' -Notice of Violation enclosed with this letter. In accordance with NRC enforcement policy, a Severity Level IV violation can be considered for nonciting if appropriate criteria are met. Violation A was ' l cited in this report because it was identified by the NRC inspector and reflects ongoing problems with surveillance procedures. Violation B was cited because it was identified by the NRC inspector and because of the potential safety significance of conducting maintenance work on - unauthorized components. This violation also reflects an additional example of failure to assure adequate clearance tag-outs for maintenance work.
