Inspection Report No. 50-271/87-04

This refers to the routine resident safety inspection conducted by Mr. W. J. Raymond

of this office on February 3 - March 2, 1987, at the Vermont Yankee Nuclear Power

Station, Vernon, Vermont.

The inspection consisted of observation of activities,

interviews with personnel and document reviews.

The results of the inspection are

described in the NRC Region I Inspection Report enclosed with this letter and were

discussed with Mr. J. P. Pelletier of your staff at the conclusion of the inspec-

tion.

Based on the results of this inspection, it appears that one of your activities

was not conducted in full compliance with NRC requirements, as set forth in the

Notice of Violation enclosed herewith as Appendix A.

The violation concerns the

failure to control maintenance and modification activities associated with the

toxic gas monitoring system on February 17, 1987 in accordance with the require-

ments of your administrative procedures for plant equipment control.

This viola-

tion has been categorized by severity level in accordance with the " General State-

ment of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix

C (Enforcement Policy 1986).

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

We note that sub-

sequent to the NRC inspection of the toxic gas monitor maintenance activity on

-February 17, 1987, you took corrective actions to control the work in accordance

with the applicable administrative procedures.

Therefore, in your response, you

need only address corrective actions taken and planned to prevent recurrence.

A second, licensee-identified violation noted during the inspection, but not cited,

is discussed in the enclosed inspection report and concerns the discovery of con-

taminated material in a dumpster outside the radiation controlled area. While the

specific incident did not create a significant hazard, we are concerned about the

recurrence of the loss of control over low level radioactive materials.

Our in-

spectors will review your corrective actions to assure that they are effective to

prevent recurrence.
