,y APPENDIX A NOTICE OF VIOLATION Vermont Yankee Nuclear Power Corporation Docket No. 50-271 Vermont Yankee Nuclear Power Station License No. DPR-28 Vernon, Vermont During an NRC inspection conducted on February 3 - March 2, 1987, a violation of NRC reluirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions", 10 CFR Part 2, Appendix C (En-forcement Policy 1986), the violation is listed below:

Technical Specification 6.5.A requires that activities affecting nuclear safety be performed in accordance with written procedures. AP 0025, Plant Equipment Control, Revision 5, dated March 31, 1986, was written pursuant to the above and established the administrative requirements that must be fol-lowed for the removal of safety related or technical specification referenced plant systems from service for maintenance or modifications.

AP 0025 requires that the operations shift supervisor authorize work activity prior to the removal of plant equipment from service to assure, in general, that plant safety or performance capability is not compromised, and that the equipment is controlled per established work control procedures during and upon completion of work activities, including the completion of post repair operability testing and verification of proper return to an operable status.

Contrary to the above, the requirements of AP 0025 were not met on February 17, 1987 when, as a result of inadequate communications between the operations shift supervisor and the work party leader during the performance of mainten-ance on the "A" toxic gas monitoring (TGM) system, the "B" TGM system was removed from service without authorization from the shift supervisor to re-place flow meters in the purge supply to the carbon dioxide reference cell.

It is noted that, subsequent to NRC inspection of the area on February 17, 1987, actions were taken by the operations shift supervisor to control the "B" TGM work activity per AP 0025.

This is a Severity Level V Violation (Supplement I.E).

Vermont Yankee Nuclear Power Corporation is hereby required to submit to this of-fice within 30 days of the date of the letter transmitting this Notice, a written statement of explanation in reply including: (1) corrective steps that will be taken to avoid further violations; and (2) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extend your response time.
