Docket No. 50-361 30-Day Report i Licensee Event Report No.82-133 San Onofre Nuclear Generating Station, Unit 2 This submittal is in accordance with the reporting requirements of Section 6.9.1.13b of Appendix A to Facility Operating License NPF-10. It describes a reportable occurrence involving Limiting Condition for Operation (LCO) 3.6.3 associated with containment isolation valves. A completed copy of LER 82-133 is enclosed.

On October 21, 1982, while in Mode 2, hot leg sample isolation valve 2HV-0509 was observed to have simultaneous open and closed Control Room indication of its position when it was, in fact, closed. The valve was declared inoperable at 1030 and LCO 3.6.3 Action Statement a invoked. As required by thi's Action' Statement, redundant valves 2HV-0508 and 2HV-0517 remained operable in their closed positions and actions were immediately initiated to return 2HV-0509 to operable status.

Subsequent investigation revealed that simultaneous valve position indication was due to a loose actuator bar on limit switch assembly 2ZSL-0509-1 attached to the valve stem. The loosened actuator bar allowed lateral movement which resulted in the bar not making contact with the open limit switch when the valve was closed. This resulted in the simultaneous valve position indication in the Control Room. After readjusting and tightening the actuator bar, the valve was declared operable at 1350 on October 21, 1982, prior to exceeding the time constraint of LCO 3.6.3 Action Statement a.

Mr. R. H. Engelken November 19, 1982 As corrective action, a Startup Problem Report (SPR) has been initiated to evaluate the feasibility of providing a key lock assembly on the valve stems of this type of valve to insure proper actuator bar tightness.

Since the hot leg sample isolation valve remained closed and its redundant valves remained closed and operable throughout the event, there was no impact on health and safety of plant personnel or the public.
