10CFR50.73(a)(2)(iv) for manual Engineered Safety Feature actuations (reactor trips) due to )

problems with main feedwater control.

l NUREG 1022, Rev 1, Section 2.3 - Multiple Failures and Related Events allows, but does not i require, "[m] ore than one failure or event [to be] reported in a single ... LER if(l) the failures or  !

events are related (have the same general cause or consequences) and (2) they occurred during a single activity (e.g., test program) over a reasonably shon time (within .. 30 days of discovery of i the first reportable event ...)."

The events reported in LERs 1999-003 and 1999-004 have the same general causes (loss of feedwater control), same general consequence (manual reactor trip), occurred during a single activity (startup following refueling), and occurred over a short time (2 days). Nevertheless, the two events are being reported separately because the actual causes are considered different and independent of each other. The two events do, however, have some common corrective actions relative to preventative maintenance and these are included in the enclosed LERs.

The health and safety of neither the public nor plant personnel were affected by this occurrence.
