Nuclear Regulatory Commission Office of Inspection and Enforcement Region V 1450 feria Lane, Suite 210 Walnut Creek, California 94596-5368 Attention: Mr. R. H. Engelken, Regional Administrator

Dear Sir:

Subject Docket No. 50-361 30-day Report Licensee Event Report No.82-046 San Onofre Nuclear Generating Station, Unit 2 Pursuant to Technical Specification 6.9.1.13.b, this letter provides the required written report and completed Licensee Event Report (LER) form for an occurrence involving Limiting Condition for Operation (LCO) 3.3-2 on the Toxic Gas Isolation System (TGIS).

On July 14, 1982, while operating in Mode 4, a Control Room alarm was received indicating f ailure of the TGIS, Train "A". Investigation revealed the pilot flame associated with the butane monitor was extinguished.

Train "A" of TGIS was declared inoperable and Action Statement 14 of Table 3.3-3 was entered.

Investigation revealed that the hydrogen fue'l supply'for the TGIS monitor was depleted and apparently resulted in burner flameout.

Immediate action was taken to replenish the fuel supply to this monitor and operability was restored within 8 hours9.259259e-5 days
0.00222 hours
1.322751e-5 weeks
3.044e-6 months
.

To avoid recurrence of such an event, operators will monitor TGIS fuel supply on a frequency of at least once per shift and will request replenishment before fuel pressure is reduced below 200 psig.

R. H. Engelken August 13, 1982 in addition, facility changes will be initiated to have at least one spare bottle of fuel connected and ready for use at all times to eliminate the delay in ordering, transporting, and connecting new fuel suppiles.

If there are any questions regarding the above, please contact me.

Sincerely,

/ H
