Reference:

Facsimile Transmission, H.B. Ray (SCE) to R.H. Engelken (NRC), Written Confirmation of Prompt Notification, dated June 10, 1982 The referenced transmittal confirmed our prompt notification of your office by telephone of an event involving incorrectly wired Auxiliary Feedwater (AFW) pump motor load shedding relays. This 14 day written follow up and License Event Report (LER) No.82-021 is provided in accordance with Appendix A Technical Specification 6.9.1.12 to Operating License NPF-10.

The incorrect wiring was caused by an error in the wiring diagram used for installation and inspection. This error resulted in the override relay contacts in the Safety injection irip circuit associated with AFW pump motors being Initially installed in the normally open position, whereas the logic diagram correctly provided for them to be in the normally closed position. The error was disclosed during the install-ation of the same relay in SONGS Unit 3 and was properly documented and dispositioned by a non-conformance report.

The prompt report was made on the basis that the incorrect wiring might have resulted in AFW pump motors remaining connected to their buses during loading of the diesel generators, contrary to design, and that this might have caused an undervoltage condition on both the 4160 and 480 volt buses.

Mr. R. H. Engelken 24 June, 1982 Our f urther evaluation has determined that operation of other relays would have prevented the AFW pump motors from overloading the diesel generators due to this incorrect wiring. Also, sequences in which the AFW pump motors are powered f rom the reserve auxiliary transformers would not have been adversely affected by this incorrect wiring.

Although this LER was due June 23, 1982, an extension on the submittal was requested in a telephone conversation on that date with D. Kirsch, Region V Operations inspector.
