NRC Region I Combined Inspection Report Nos. 50-220/90-06 and 50-410/90-06 This letter transmits the results of the routine inspection conducted by Messrs. W. Cook, R. Laura, and R. Temps of this office on May 31 through July 12, 1990, at Nine Mile Point Units 1 and 2, Scriba, New York. A summary of the findings was presented to Mr. J. Firlit and other members of your staff at the conclusion of the inspection.

One violation with two examples was identified during this period concerning improper control of equipment status through your Blue Markup control process, and a Notice of Violation (Appendix A) is attached. This violation has been categorized as Severity Level IV according to 10 CFR Part 2, Appendix C (Enforcement Policy). This violation concerns failures of your, staff to main-tain positive contr~1 of system status. It also identifies an incomplete root cause analysis following the May 21, 1990 feedwater pump suction valve event, in that the correction actions did not identify additional Blue Markup con-trolled components which were out of position and were subsequently discovered by the inspectors. We are concerned that the root cause analysis focused on personnel performance and did not evaluate the effectiveness of the tagging process.
