dated July 2, 1982 The referenced letter confirmed our prompt notification of your of fice of our failure to provide Licensee Event Reports (LER's) within 30 days of the dates upon which the event had occurred.

4 Pursuant to Technical Specification 6.9.1.13.b, to Operating License NPF-10 for San Onofre Unit 2, this submittal provides the required 30 day written report and copy of LER form, for an event involving the i

Mainsteam Atmospheric Dump Valve.

On June 10, 1982, while in Mode 3, steam was observed leaking through the Mainsteam Atmospheric Dump Valve, 2HV-8421, with the valve in the indicated full closed position.

In accordance with Technical Specification 3.6.3, this valve was declared inoperable. The associated Action Statement c requires that isolation of the affected penetration, by use of at least one closed manual valve or blind flange be achieved within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br />. This was accomplished by closing the upstream manual block valve, S21301MU001. The inoperability of valve 2HV-8421 was investigated by l&C and Mechanical Maintenance personnel. Af ter trouble shooting and i

cycling of the valve and actuators, the valve was returned to service.

Mr. R. H. Engelken July 22, 1982 To establish operability, the section of the Semi-Annual Subgroup Relay Test, (Operator Surveillance Test S023-3-3.43) which tests the controller, relays, actuator and 2HV-8421 valve functions, was performed. All components functioned properly and operability was reestablished within 18 hours2.083333e-4 days <br />0.005 hours <br />2.97619e-5 weeks <br />6.849e-6 months <br /> of the event identification.

A completed LER form is enclosed.
