Subject: Docket-No. 50-361 30-Day Report Licensee Event Report Nos.62-066 and 82-067 i -, San Onofre Nuclear Generating Station, Unit 2 qr Pursuant to Technical Specification 6.9.1.13b to Operating License NPF-10 for San Onof re Unit 2, this submittal provides the. required 30-day written reports and Licensee Event Reports-(LER's)82-066 and 82-067 4

for events involving the Auxiilary Feedwater System (AFWS).

in the first event on July 19, 1982 at 2316, whlie in Mode 3, an operator attempting to open valve 2HV4706 noted that the valve would not open.

Consequently, one AFW train associated with the turbine driven pump 2P-140 was declared inoperable and Limiting Condition for Operation (LCO) 3.7.1.2 Action "a" was ' initiated. This action "a"' requires that the train be restored to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />.

Inspection of the valve revealed that the anti rotation plate / mechanical indicatgr :nas bent and jammed against the. yoke guides with the valve in the<ct'osed position. This incident occurred because the range of travel of the va.lve actuator was not set correctly and permitted the plate to move a63ve the yoke guides'and rotaie. Subsequent closing of the' valve deformed and jammed the plate against.the yoke guides.

August-18, 1982' Corrective measures included the inst'allation of'new yoke' guides to.

prevent future ? Jamming; and the replacement of the damaged anti-rotation plate.

LER 82-066 addresses this event. These actions were completed and the AFW train restored to operable -status on-July 21, 1982 at 1745,-

within the allowable 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />.

In the second event on July 20, 1982 at 0620, while in Mode 3, Valve 2HV4713 'f alled to open.when an operator attempted to open it from the Control Room.. The cause for this failure was a loose coupling lock nut

~which allowed the actuator shaft to separate.from the valve stem.

Consequently, the AFW train associated with.the motor driven pump 2P-141 was declared inoperable and LCO 3.7.1.2 Action "b" was initiated, since the first train was still inoperable. Action "b"

requires that the plant be in at least Hot Standby (Mode 3) within 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in Hot

Shutdown (Mode 4) within the following 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br />.

Corrective action included the installation-of a new' anti-rotation plate / mechanical Indicator and reassembly of the coupling..LER 82-067 addresses this event.. The repair was completed at 1130 am on July 20,

- 1982.

There was no consequence to public health and' safety as a result of these events since at least one.100% AFW train was always available to perform the decay heat; removal function if called upon +o do so.
