- Detector, 2-VRA-2310, wasinoperable longerthanallowedbytheActionStatement.
OnJuly10,1993at1330hours,thedetectorwasdeclaredinoperable duetotheinability toperformthechannelchecksrequiredbythesurveillance requirements.
Althoughthedetectorwasstillfunctional, itwasconsidered anddeclaredinoperable.
Theredundant instrumentation wasconfirmed tobeoperable.
However,theSpecialReportrequiredbyTS3.3.3.1wasnotsubmitted totheNRCwhenthedetectorremainedinoperable inexcessofthe7dayActionStatement requirement.
ThefailuretocomplywiththeSpecialReportrequirement wasdiscovered onJuly30,1993,andisrectified bytransmittal ofthisletter.Thiseventresultedfromtherepairactionnotbeingprioritized inaccordance withtheapplicable LCOrestoration requirement, coincident withthefailuretoidentifytheneedforaSpecialReport.Thisisattributed to:1)ineffective communication oftheTSActionStatement requirement viatheWorkRequest,.
