On July 7,
- 1984, a portion of the Unit 1 inside fire protection ring header was removed from service to facilitate repairs on three valves in the Fire System.
This isolation removed numerous non-Technical Specification Fire Hose Standpipe and Suppression Systems from service, as well as one Technical Specification fire hose standpipe.
The requirements of Technical Specification 3.7.9.5, Fire Hose Stations, were met in that an additional fire hose of ecpxivalent capacity was routed to the area of the inoperable Technical Specification
,fire hose standpipe within one hour, and the entire Fire Protection System was returned to service within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.
The requirements of Technical Specification 3.7.9.1 Action b2a were met in that a verbal report was made by telephone to the NRC Operations Center within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> of the isolation.
- However, due to a weakness in our administrative tracking procedures, which existed at that time, and the fact that the incident was internally categorized as non-reportable, the follow-up reports as required by Technical Specification 3.7.9.1 Action b2b and c were never made.
Following a recent review of this incident, it was determined that a follow-up report should be filed.
Prior to discovery of this reporting error, we had already implemented internal policies that will more effectively track system impairments and reporting recpxirements and will help to minimize the chances of this type of error happening in the future.
