Dear Mr. Davis:

This special report is being submitted in accordance with Technical Specification 3.7.10 action a, to inform you that the following seven fire barriers have been inoperable longer than allowed by Technical Specification 3.7.10 action

'ire barrier penetration seal W-7109, located in the Unit One 4KV complex, was declared inoperable on January 2,

1990 due to a destructive examination of the seal.

When the seal was removed, it was determined that an engineering evaluation was necessary to repai" the seal due to an inaccessible gap under the cable tray which passes through the fire barrier penetration.

We will inform you through a follow up report when the engineering evaluation is complete and the fire seal has been restored to operability.

A fire watch, as required by Technical Specification 3.7.10 action a, is being maintained while the seal is inoperable.

Fire door 344, the Unit Two 4KV complex rollup fire

door, was declared inoperable on January 6,

1990 due to the failure of its rolling track mechanism.

It was repaired and restored to operability on January 17, 1990.

A fire watch, as required by Technical Specification 3.7.10 action a was maintained while the door was inoperable.

Fire door 388, the Unit Two Reactor Cable Tunnel.

Quadrant N2 double door, was declared inoperable on January 8,

1990 due to the failure of its door closure sequencer.

Due to the nature of the door hardware failure, it was necessary to special order new doors to replace the existing ones.

It is anticipated that the new doors will be, received and installed by May 25, 1990.

A fire watch, as required by Technical Specification 3.7.10 action a will be maintained while the door is inoperable.

Fire door 371, the Contractor Access Building to Auxiliary Building door was declared inoperable on January 13, 1990 due to the failure of its latching mechanism.

The parts needed to repair the latching mechanism have been ordered.

It is anticipated that the parts'ill be received and the door restored to operability by April 2, 1990.

A fire watch, as required by Technical Specification 3.7.10 action a, is being maintained while the door is inoperable.

Fire barrier penetration seal W-5080, separating the Unit One Auxiliary Cable Vault and the 620'levation of the Auxiliary Building, was declared inoperable on January 16, 1990 due to a 1/4 inch gap between the top of the sealant material and its penetration.

We are presently awaiting delivery of materials necessary to repair the seal.

It is anticipated that the fire seal will be repaired and restored to operability by April 2, 1990.

A fire watch, as required by Technical specification 3.7.10 action a, is being maintained while the seal is inoperable.

Zone 28 fire detection in the Unit One East Main Steam Enclosure was declared inoperable on January 16, 1990 due to.,purious alarms.

Due to the intermittent. nature of this malfunction, each branch of the detection circuit is being individually isolated and observed in order to identify the faulty detector.

It is anticipated that the Zone 28 detection will be restored to operability by March 17, 1990.

Although this fire detection circuit is not contained in our current Technical specifications, it is included in a proposed Technical Specification amendment pending approval.

Therefore, we are taking administrative actions in accordance with the proposed amendment.

A fire watch will be maintained while the detection is inoperable.

Fire barrier penetration seal W-5093 was declared inoperable on January 24,

1990, due to inadqueate depth of sealant material.

It was repaired and restored to operability on February 1,

1990.

A fire

watch, as required by Technical Specification 3.7.10 action a, was maintained while the seal was inoperable.
