Dear Mr. Davis:

This letter is to inform you that six fire barriers have been inoperable longer than the seven days allowed by Technical Specification 3.7.10 Action a, and to serve as a follow up report as committed to in a previous letter.

On February 17, 1989, while conducting a routine inspection, a

Quality Control Technician discovered a gap between the top of the removable walls and the permanent structure separating Fire Zones

63A, 63B, and 63C from Fire Zone 5.

Fire Zones

63A, 63B and 63C are the Unit Two Charging Pump Rooms and Fire Zone 5 is the general East/West Corridor of the Auxiliary Building 587'levation.

This inspection also revealed that gaps existed between the removable blocks and numerous conduits that pass through the removable portion of the wall.

After finding these conditions on the Unit Two removable walls, the inspector checked the removable walls separating Fire Zones

62A, 62B, 62C, the Unit One Charging Pump Rooms, and Fire Zone 5.

Gaps were also discovered between the top of the removable walls and the permanent structure.

At that time the removable walls in both units were declared inoperable and a roving fire watch, as required by Technical Specification 3.7.10 Action a, was established for the affected fire zones.

This roving patrol will remain in effect until the problems are resolved.

Due to the unique nature of these barriers an engineering evaluation and determination on how to address the gaps has been requested.

It is expected that the evaluations and repairs will be completed by July 31, 1989.

In a letter dated December 22, 1988 we informed you that Fire Door 346 (separating Fire Zones 45 and 46A), Fire Damper 1-HV-TX-FD-1 (separating Fire Zones 14 -and 79),

and Fire Damper 2-HV-TX-FD-1 (separating Fire Zones 20 and 85) were inoperable.

Following is an update on the information provided in our December 22, 1988 letter.

Fire Door 346 was repaired and returned to service on March 20, 1989.

Fire Dampers 1-HV-TX-FD-1 and 2-HV-TX-FD-1 remain inoperable because they will not close against the ventilation air flow during a fire.

We have determined that an acceptable solution to this is to procedurally require the manual shutdown of the ventilation system in the event of a fire in the area.

To achieve this, the annunciator response procedures for the fire detection in these fire zones will be changed to require the manual shutdown of the ventilation systems upon actuation of the fire detection and verification of a fire in these areas.

These changes will be completed by April 30, 1989.
