Dear Mr. Davis:
This special report is being submitted in accordance with Technical Specification 3.7.10 Action a, to inform you that the following eight fire barriers have been inoperable longer than the seven days allowed by Technical Specification 3.7.10.
Fire barrier penetration seal
- F6185, separating the Unit One control room and the Unit One control room cable
- vault, was declared inoperable on August 22, 1989 and was restored to operability on August 30, 1989.
A fire
- watch, as required by Technical Specification 3.7.10 Action a, was maintained while the seal was inoperable.
Fire dampers 1-HV-ACF-Dl and 1-HV-ACF-D2, in the Unit One P-250 computer room; and fire dampers 2-HV-ACF-Dl and 2-HV-ACF-D2, in the Unit Two P-250 computer room
,were declared inoperable on August, 21,. 1989 due to a failure of the fire damper release mechanism associated with each damper.
At that time, a 'roving fire watch, as required by Technical Specification 3.7.10 Action a was established for the affected fire zones.
This roving patrol will remain in effect until the problem is resolved.
An engineering analysis/evaluation is being performed to determine whether a reconfiguration of the fire damper release mechanisms is necessary.
We will inform you through a follow-up report when the engineering analysis/evaluation has been completed and the dampers have been restored to operability.

Fire damper 1-HV-AS-D2A, in the 633'uxiliary Building
- hallway, was declared inoperable on August 22,'989, and was restored to operability on September 8,
1989.
A Fire Watch, as required by Technical Specification 3.7.10 Action a, was maintained while the fiie damper was inoperable.
Fire damper 1-HV-AS-D1A, in the 633'uxiliary Building hallway was declared inoperable on August 29,
- 1989, due to a failure of its closing tension mechanism.
Repair work has been temporarily suspended on this damper due to another ongoing plant modification which reauires that the damper remain open for plant ventilation purposes.
It is anticipated that the plant
- modification will be completed and damper 1-HV-AS-D1A restored to operable status by October 30, 1989.
A fire watch, as required by Technical Specification 3.7.10 Action a; was established and will be maintained until the damper. is restored to operability.
Fire damper 2-HV-AC-FD-3, in the Unit Two P-250 computer room was declared inoperable on September 6,
- 1989, due to the failure of its fire damper release mechanism.
It is anticipated that the damper release mechanism will be repaired and the damper restored to operability by October 23, 1989.
A fire watch, as required by Technical Specification 3.7.10 Action a, was established and will be maintained until the damper is restored to operability.
