Dear Mr. Davis:

This special report is being submitted in accordance with Technical Specification 3.7. 10 Action a, to inform you that several fire barrier penetration seals have been inoperable'onger that the seven days allowed by Technical Specification 3.7.10 Action a.

Fire Barrier Penetraton Seals F-6714 and F-6706 separate the Unit 2 Control Room and Unit 2 Contxol Room Cable Vault.

Fire Seal F-6714 was declared inoperable on April 9,

1989, and returned to service on May 2, 1989.

Fire Seal F-6706 was declared inoperable on April 11,

1989, and returned to service on April 23, 1989.

Fire watches, as required by Technical Specification 3.7.10 Action a, were maintained while the seals were inoperable.

The fire seal under Motor Control Center 1 AMD (fire seal not numbered) in the Unit 1 Auxiliary Building was declared inoperable on April 19,

1989, and returned to service on April 28, 1989.

A fire watch, as required by Technical Specification 3.7.10 Action a, was maintained while the seal was inoperable.

The TSI fire retardant. material was removed from conduit 98676-1 and electrical pull box B-41 in the Unit 1 ESW Pump Room on April 17, 1989.

The TSI was reapplied on April 26, 1987.

TSI is a fire retardent material applied to specified cable trays and conduits for Appendix R purposes.

A fire

watch, as required by Technical Specification 3.7.10 Action a, was maintained while the TSI material was removed.

Fire Door 346 was declared inopexable on April 14,

1989, when it failed to close completely during routine testing.

Following repair the door was returned to operable status on April 26, 1989.

A fire watch, as required by Technical Specification 3.7.10 Action a, was maintained while the door was inoperable.

Fire Doors.349, 371 and 347 in the Unit 1 4KV complex and 350 and 472 in the Unit 2 4KV complex were declared inoperable on April 14,

1989, as the result of an adverse trend of fire doors failing to close from the open position during CO2 system testing.

A fire watch, as recpxired by Technical Specification 3.7.10 Action a, was established at that time and will remain in effect until the problems can be resolved.

Engineering evaluations are under way to determine a permanent resolution.

We will notify you when a schedule for completion can be determined.

In a letter dated October 29,

1988, we informed you that.

Fire Door 371, separating the New Containment Access Building and the Aux Building Crane Bay, was inoperable.

Following extensive reworking of the door frame and the door hardware, the door was returned to service on April 26, 1989.
