SUBJECT: REPORT NOS. 50-327/86-19 AND 50-328/86-19 Thank you for your response of August 19, 1986, to our Notice of Violation issued on May 13, 1986, concerning activities conducted at your Sequoyah facility. We have evaluated your responses and found that they meet the requirements of 10 CFR 2.201. We will examine the implementation of your corrective actions during future inspection Regarding your request for further clarification of Violation 86-19-12, TVA has been cited for not meeting the operability / action requirements of the stated Technical Specifications (TS). It is recognized that operations personnel were unaware of the existing problem. However, unlike the situation where an equip-ment malfunction is not known until requisite testing is performed, the events which led to both hydrogen analyzer trains being inoperable were a direct result of TVA actions. These actions, if properly controlled, would have p.evented said condition. Accordingly, stated corrective actions for Violation 86-19-06 have been determined to be appropriate for Violation 86-19-12 as well. Therefore, no further response regarding this matter is require With respect to the portion of Violation 86-19-01 concerning the hydraulic lock release valves, it was recently determined that these valves are equipped with a set screw type mechanism that has never been utilized by TVA. This aspect will be addressed in future correspondence with TVA concerning surveillance testing of the upper head injection system isolation valves. Accordingly, NRC agrees that the violation as stated is inappropriate and withdraws that portion of the violatio We appreciate your cooperation in this matte
