Gentlemen:

SUBJECT: NOTICE OF VIOLATION (INSPECTION REPORT NO. 50-416/86-14)

This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by

W. T. Cooper on May 5-9, 1986. The inspection included a review of activities

authorized for your Grand Gulf facility. At the conclusion of the inspection,

the findings were discussed with those members of your staff identified in the

enclosed inspection report.

Areas examined during the inspection are identified in the report. Within these

areas, the inspection consisted of selective examinations of procedures and

representative records, interviews with personnel, and observation of activities

in progress.

The inspection findings indicate that certain activities appeared to violate NRC

requirements. The violation, references to pertinent requirements, and elements

to be included in your response are described in the enclosed Notice of

Violation.

During an inspection conducted on July 8-12, 1985 (Inspection Report

No. 50-416/85-24), an unresolved item was identified concerning the experience

level of the individual in the position of Chemistry / Radiation Control (C/RC)

Superintendent. Unresolved items are matters about which more information is

required to determine whether they are acceptable or may involve violations or

deviations. Subsequent to that inspection, you agreed to provide training and

work assignments for the individual such that he would acquire additional

experience in supervising a commercial nuclear power plant radiological

protection program. In addition, you took compensatory measures to ensure that

during this training the plant's radiological protection program was being

supervised by an individual who fully met the qualification requirements of the

Technical Specifications. On March 18, 1986, Technical Specification 6.3.1 was

amended to permit the continued use of the compensatory measures until the

C/RC Superintendent fully met the experience requirements of the Technical

Specifications. The NRC considers that, at the time of the July 8-12, 1985,

inspection, the C/RC Superintendent did r:ot fully meet the requirements of

Technical Specification 6.3.1. However, because of your good faith performance

in addressing this issue, Regional management has determined that this item will

not be included in the enclosed Notice of Violation and that the unresolved item

will be considered closed.

Title 10, Code of Federal Regulations, a copy of this letter and its enclosures

will be placed in the NRC Public Document Room.

The responses directed by this letter and its enclosures are not subject to the

clearance procedures of the Office of Management and Budget as required by the

Paperwork Reduction Act of 1980, PL 96-511.
