O ENCLOSURE 1 NOTICE OF VIOLATION Mississippi Power and Light Company Docket No. 50-416 Grand Gulf License No. NPF-29 During the Nuclear Regulatory Commission (NRC) inspection conducted on May 5-9, 1986, a violation of NRC requirements was identified. The violation involved failure to meet Technical Specification requirements for locking high radiation areas. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violation is listed below:
Technical Specification (TS) 6.12.2 requires that areas accessible to personnel, with radiation levels such that a major portion of the body could receive in one hour a dose greater than 1,000 millirem, shall be provided with locked doors to prevent unauthorized entry. This TS also requires doors to remain locked except during periods of access by personnel under an approved RWP.
Contrary to the above, the requirement to provide locked doors to prevent unauthorized entry into very high radiation areas was not met in that on June 19, 1985, August 8,1985, and January 14, 1986, doors providing access to the turbine bioshield area were found unlocked.
Dose rates in the area exceeded 1,000 millirem per hour.
This is a Severity Level IV violation (Supplement IV).
3 Pursuant to the provisions of 10 CFR 2.201, Mississippi Power and Light Company is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including for each violation: (1) admission or denial of the violation, (2) the 4
reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending the response time.
