SUBJECT: NRC INSPECTION REPORT NO. 50-416/99-03
Dear Mr. Eaton:

This refers to the inspection conducted on April 5 to 8,1999, at the Grand Gulf Nuclear Station
facility. The purpose of the inspection was to review your facility's corrective action program,
using the guidance provided in NRC Inspection Procedure 40500. An exit meeting with your
staff was held on April 8,1999. A supplemental onsite inspection was conducted from May 10
to 11,1999, which concluded with a briefing for representatives of your staff. Inoffice inspection
of this supplemental information was performed through June 28,1999. Following the inoffice
inspection, an exit was conducted telephonically on July 22,1999, with your staff. The
enclosed report presents the scope and results of the inspection.
On the basis of the sample reviewed, your corrective action program was acceptably
implemented, with senior management involvement and oversight being maintained through
routine reports and meetings. During the review of condition reports and other associated
corrective action documents, the inspectors observed examples of slow or untimely closure of
the associated condition report and one example of a failure to implement prompt corrective
actions. Similar issues were identified in NRC Inspection Report 50-416/99-02. Additionally,
self-assessments conducted by your staff identified related problems involving examples of

multiple extensions of root cause analyses and late closures of corrective actions.
During this inspection, we also noted that the use of temporary or interim solutions may result in
the f ailure to appropriately implement established design and work control measures. In some
instances, temporary or interim engineering dispositions to accept-as-is material deficiencies
and interim repairs were not effectively tracked by the corrective action program.
In addition to questions regarding the implementation of temporary or interim solutions, other
recent inspection reports have raised questions regarding the scope and rigorousness of a
number of other programs. These include the updated final safety analysis report review and
update project, technical specification instrument setpoint calculations (as documented in NRC
Inspection Report 50-416/99-02) and the surveillance credit program (as documented in NRC
Inspection Report 50-416/99-06, Section F2.2).

As a result of these issues, and as noted in NRC Inspection Report 50-416/99-02, we des;re to
meet with you in the Region IV office to gain a better understanding of your perspective
regarding these findings and programs, including any plans you have to enhance these
programs and their implementation. We will contact you regarding a mutually agreeable date
and agenda for this meeting.
Based on the results of this inspection, the NRC also determined that a violation of NRC
requirements occurred for untimely corrective actions. This violation is being treated as a
noncited violation, consister,t with Appendix C of the Enforcement Policy. The noncited
violation is described in the subject inspection report. If you contest the violation or severity
level of the noncited violation, you should provide a response within 30 days of the date of this
inspection report, with the basis of your denial, to the Nuclear Regulatory Commission, ATTN:
Document Control Desk, Washington DC 20555 0001, with copies to the Regional
Administrator, Region IV, and the Director, Office of Enforcement, United States Nuclear
Regulatory Commission, Washington, DC 20555-0001; and the NRC resident inspector at the
Grand Gulf Nuclear Station.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its
enclosure will be placed in the NRC Public Document Room (PDR).
