SUBJECT: NRC INSPECTION REPORT NO. 50-416/99-02

Dear Mr. Eaton:

This refers to the team inspection conducted on February 22-26 and March 8-12,1999, at the

Grand Gulf Nuclear Station facility. A supplemental exit meeting was conducted by telephone

on May 18,1999. The enclosed report presents the results of this inspection. The

inspection involved a safety system engineering inspection of the three trains of emergency

diesel generators and the associated switchgear and ventilation, as well as a review of your

10 CFR 50.59 safety evaluation program, engineering staffing and backlog, and previous NRC

inspection findings.

The inspection did not identify any operability concerns affecting the emergency diesel

generators or other equipment but did reveal a number of problem areas. One of these

involved the team's identification of multiple discrepancies in the Updated Final Safety Analysis

Report. The affected sections had been previously reviewed by your contractor, and most of

the discrepancies identified by the team had not been identified during this contractor effort.

This number of identified discrepancies was indicative of a potential weaknesses involving the

overall fidelity of the Updated Final Safety Analysis Report and the adequacy of the Updated

Final Safety Analysis Report review and update project.

The second concern involved the lack of documentation supporting instrument setpoints. No

calculations existed to support many of the setpoints delineated in the technical specifications.

Calculations existed for some setpoints, but they typically did not address loop uncertainties or

sensor type and installation. Although we were generally able to conclude that the reviewed

setpoints were conservatively set, our review was limited to the emergency diesel generators

,

and related support systems. Consequently, this issue will remain unresolved pending further

NRC review.

The third concern involved the implementation effectiveness of the corrective action program.

The team identified that the corrective actions taken in response to two previous violations (one

involved past Updated Final Safety Analysis Report discrepancies) and several condition

reports did not address or identify the causes of the problems, were limited in scope, or were

not fully effective in correcting the problem. Additional corrective action issues noted by the

team included the Updated Final Safety Analysis Report discrepancies discussed above, the

prolonged period involved in resolving potential concerns regarding the ability of certain motor-

operated valves to open and close against design basis differential pressure and flow, as well

as, a !ack of documented justification for extending the corrective action due dates for a number

of temporary alterations.

Additionally, this and other recent NRC inspections have raised questions regarding the

scope or appropriateness of a number of other programs. These include the locked valve

program (Section E1.1.3 of this report), the temporary / interim solution program (refer to NRC

Inspection Report 50-416/99-03), and the surveillance credit program (refer to NRC Inspection

Report 50-416/99-06).

As a result of these issues, we desire to meet with you in the Region IV office to gain a better

understanding of your perspective regarding these findings and programs, including any plans

you have to improve your programs and their implementation. We will contact you regarding a

mutually agreeable date and agenda for this rneeting following the issuance of NRC Inspection

Reports 50-416/99-03 and 50-416/99-06.

Based on the results of this inspection, the NRC has determined that two violations of NRC

requirements occurred. These violations are being treated as a noncited violations, consistent

with Appendix C of the Enforcement Policy. These noncited violations are described in the

subject inspection report. If you contest the violations or severity level of these noncited

violations, you should provide a response within 30 days of the date of this inspection report,

with the basis for your denial, to the Nuclear Regulatory Commission, ATTN: Document

Control Desk, Washington DC 20555-0001, with a copies to the Regional Administrator,

Region IV, and the Director, Office of Enforcement, United States Nuclear Regulatory

Commission, Washington, DC 20555-0001.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its

enclosure will be placed in the NRC Public Document Room (PDR).
