ATTACHMENT Special Renart No.92-003. Units 1 and 2 Inoperable Fire Surpression Water Sv$t9@

On July 15, 1992, a section of pipe in the fire main yard loop ruptured causing the fire protection water tar.k leves to drop below the level required by Technical Specification 3.7.11.1 and the loss of fire main water pressure.

It 1323, the control room received a fire main low pressure alarm and noted the start of all fire pumps. A',1326, the control room received the low level alarm on the fire protection water tanks. At l'*31, it was determined that no fire existed, and all fire pumps were secured a conserve water. The lowest level reached in the water tanks wac 24 feet. The fire main rupture was isolated at 1403. Areas affected by this isolation were provided with sufficient additional lengths of hose to back up the isolated hose houses within one hour as equired by Technical Specifications, The fire pumps were restored, and the fire water tank levels were recovered to the Technical Specification limit at 1455. All Technical Specification required actions associated with 3.7.11.1 were met.

The ruptured section of pipe was removed and replaced. The damaged section of pipe is being examined to determine the cause of the rupture. The results of >

the examination are expected by July 31, 1992. The piping was replaced on July 20, 1992, at 1444 and restored to operable status on July 23, 1992, at 1700.

For thl's event, Technical Specification 3.7.11.1 requires the following Special Reports: .a) notification by telephone within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />; b) confirmation by telegraph, mailgram or facsimile no later than the next working day; and c) in ,

writing within 14 days following the event. Telephone notification was made on July 15. 1992, and a facsimile report was made on-July 16, 1992. This Special Report er..pletes the notification requirements of Technical Specification 3.7.11.1.
