Enclosure Special Report No.'89-002, Unit 1 Fire Damper Inoperable Due to Failure to Close With Air Flow On 6-15-89,,during the performance of fire damper testing, damper 1-100-113-05 did'not meet the acceptance criterion of closing with air flow in the system.

This. damper is.in a ventilation system that has both winter and summer modes of-operation. The summer mode has a different air flow arrangement than the winter mode. Previous testing in the fall of 1987 had verified that this damper would close properly with the system in the winter mode of operation but, due to a. procedural inadequacy, the system was not tested in all modes'of operation. Recent testing has shown that the. damper will not close under the conditions imposed by the summer mode of operation.

. Design changa 89-1-5925 has been initiated to determine how this damper should be modified or replaced to ensure proper. operation under all system conditions.

This is expected to be' completed within the next nine months.

As noted in the special Report dated 7-5-89 (LER 89-003-00), five fire dampers-failed to close when tested. An investigation found that the affected systems. l had both winter and summer modes of operation. Previous testing had shown that the dampers would close when tested in the winter mode of operatio" .During j

subsequent testing, damper 1-100-113-05 failed to close. This damt testing program is being revised to assure that fire dampers in safety related structures will operate satisfactorily in all design operational conditions.

The revisions will consist of the following:

1. Drawings will be reviewed to determine which systems have more than one

-operational mode.

2. For those systems which have more than one operational mode, dampers will be retested under the maximum flow conditions for each mode of operation.

This retesting program will be performed on an expedited basis. Repairs or modifications will be accomplished as required.

Technical Specification 3.7.12 requires this damper to be returned to operable status within seven days or a Special Report must be submitted within the following 30 days. Therefore, this Special Report is being submitted. All Technical Specification action statement requirements for this fire damper are being met.
